Factors influencing NI in real-world needs

Addressing the healthcare needs of older adults at the Philippine General Hospital (PGH) requires effective use of nursing informatics. Research indicates that older adults face increasing unmet needs related to accessibility, availability, and acceptability of geriatric care services due to a fragmented system. Nursing informatics can improve care delivery by integrating electronic health records (EHRs) and patient management systems tailored for older adults.

Given that older adults often struggle with digital tools due to unfamiliarity with technology, it is crucial to develop user-friendly systems. Tailored solutions, such as simplified interfaces and dedicated support, are essential to bridge the digital divide. Additionally, training healthcare workers to use these tools effectively and to engage with older patients is necessary.

Data management through nursing informatics can also help identify service gaps and optimize resource allocation. Patient portals and mobile apps can enhance patient engagement by providing accessible health information and facilitating communication with care providers.

You’re right, Rainier! When I first learned that healthcare facilities are increasingly favoring informatics for managing health services, such as encouraging online consultations over walk-ins, I also thought about older people. Many of them may not be very adept with technology, and this shift could pose challenges for them in accessing healthcare.

‌The specific group I chose would be individuals suffering from mental disorders. There are a few factors that influence the application of nursing informatics in the delivery of healthcare services. First and foremost, the accessibility. Although the use of technology is rampant in this modern age, not every individual is equipped with proper gadgets or has access to the internet to avail mental health services. Additionally, the cost of implementing specific nursing informatics can be a hindrance despite the potential benefits it would bring to the table. Lastly, the competence of the healthcare provider in handling technology should be factored in, as there might occur gaps in delivering care.

Critical care for high-risk general ward patients can be delivered efficiently using nursing informatics, specifically pulmonology patients from rural to secondary facilities. The study highlighted that due to COVID-19, there has been an influx of patients who seek tertiary care. In the said country, South Korea, these times have led them to focus on patients that were critically ill. To improve their healthcare system, they used an integrated monitoring system and information system to deliver care to patients safely and efficiently and also to ensure the efficiency of the healthcare team as a response. A shift towards innovative healthcare is important to ensure quality management and control along with data utilization which is a vital part in the technology needed to monitor high risk patients.

As the study aims, nursing informatics can enhance real-time patient monitoring, immediate response to emergencies, and health safety promotion. This includes the completion rate of the system and the data linkages connecting the monitor of patients and ventilators to EMR and dashboards. Shortly speaking, it was a wireless technology that was helpful in the improvement of monitoring efficiency. The study mentioned how the implementation of a critical care monitoring system can significantly lessen the annual Code Blue incidents into half when real-time monitoring and alerts are done at nursing stations.

Through nursing informatics, high-quality care can be delivered efficiently while protecting patient safety due to rapid response attributed to real-time monitoring which can affect the work efficiency of the medical staff and lead to better patient outcomes.

In my opinion, a priority healthcare needed in my community is mental health services for young adults, particularly those between the ages of 18 and 25. Based on research, even people around this age group are vulnerable to mental health issues, such as anxiety, depression, and substance abuse, which can have long-term consequences on their well-being and productivity. Unfortunately, many young adults in my community lack access to effective mental health services, leading to poor health outcomes and increased healthcare costs.

Two underrated factors that I think influence the application of nursing informatics in the delivery of healthcare services are data privacy and security and cultural and linguistic barriers. This is because, despite the accessibility of technology,  mental health information is highly sensitive, and patients may be hesitant to share their personal data due to concerns about privacy and security. Nursing informaticists must ensure that electronic health records (EHRs) and other health information systems are secure and compliant with data protection regulations. Another is the fact that the Philippines is culturally and linguistically diverse, and mental health services must be tailored to meet the unique needs of each group. Therefore, we must develop culturally sensitive and language-appropriate health information systems and resources.

We share the same insights, Therese! I agree that a priority healthcare need is better mental health services as this is a serious and sensitive topic that wasn't really given any focus on. With this, I liked how you mentioned that EHRs should be secured and nursing informatics can surely help with that! I'd also like to highlight your acknowledgment of delivering a culturally sensitive and language-appropriate care since this is usually disregarded. By doing so, this will surely be able to create an environment that enables these vulnerable individuals to feel a sense of warmth and trust in the healthcare professionals, improving the overall quality of health.

Since more people are becoming open to recognizing mental health illnesses, this is the specific group I will be focusing on. Given the sensitivity of mental health information, data privacy and security are critical considerations. Patients may be hesitant to use digital tools if they are concerned about confidentiality breaches or the stigma connected with their mental health data. As a result, privacy measures are critical for building confidence and encouraging patient participation. Additionally, digital literacy and patient participation play an important role. While informatics technologies can help patients manage their mental health through self-monitoring and digital reminders, some patients, especially those with cognitive impairments or severe mental health issues, may struggle to navigate these systems. Nonetheless, Edgcomb (2021) highlights that health informatics, especially the analysis of electronic health record data, helps identify key areas for quality improvement, reveals disparities in mental health service access and delivery, and uncovers predictive factors for populations at risk of adverse outcomes like suicide and psychiatric hospitalizations.

A critical healthcare need within our community is improving the quality of postpartum care, particularly the discharge instructions provided to new mothers. Research indicates that a significant gap exists in this area, with 67% of postpartum nurses spending less than 10 minutes on discharge instructions related to postpartum care, warning signs, and newborn care (Morris et al., 2021). This leads new mothers unprepared to navigate the postpartum period, prompting many of them to seek information online. However, many women report that there is a lack of relevant and evidence based information that is accessible.

To address the need of improving postpartum care several factors must be considered in utilizing nursing informatics effectively. First, improving technological access should be a priority as many areas, especially in rural regions may have limited digital resources and connectivity. Hindering them from having an opportunity to talk with a healthcare provider whenever they have questions or just access information in general. Second, we can offer guidelines and alerts that help healthcare providers to be able to deliver timely and accurate care with the help of the evidence-based informatics system. For instance, we can provide access to standardized guidelines for postpartum mothers so that they are knowledgeable on the do’s and don’ts. Lastly, by utilizing digital communication tools such as messages or mobile apps, we can enhance patient engagement. It can also be utilized in giving reminders, updates, and educational content to new mothers ensuring that they receive the support and guidance that they need.

By doing these, nursing informatics can greatly impact on how we can improve postpartum care here in the Philippines, ultimately supporting new mothers efficiently and effectively.

One of the difficulties that geriatric patients have is that most informatics tools, in the form of technology, are not fitted to their unique needs. Their access to relevant health information is crucial for them to make informed choices for their health. Sadowski (2010) highlighted the importance of providing older people with information that will guide them in making decisions regarding their health. With this, nursing informatics can aid this by adjusting the complexity of technology.

The theory of diffusion of innovations by Rogers states that one attribute that can affect the adaptation of innovation is their complexity—the more complex, the less adaptability. In this case, considering that older people grew up without technology, the complexity can hinder older people from benefiting from informatics tools. With this, adjustments or even creating of informatics tools that requires no additional skills will benefit geriatric patients in the long-term. Scheerens et al. (2021) developed a study that aimed to create eHeatlh tools. In their study, one of the ways to address the complexity of eHealth tools is by adjusting the design and layout to ensure ease of use. Ensuring an increase in the accessibility and a decrease in complexity, geriatric patients will become more motivated to access credible health information.

Great points, Matthew! My heart goes out to the elders of our community. Indeed, complexity is a barrier for many geriatric patients when accessing health information. Nursing informatics can play a crucial role in simplifying these tools to meet their unique needs. Reducing complexity, as you mentioned, is key to encouraging adoption among older adults, as outlined by Rogers' diffusion of innovations theory.

Also, in addition to simplifying the design, providing ongoing support and education for geriatric patients is essential. A lot of applications and devices for our elders are actually being developed, but I noticed that they still struggle with technology, even if simplified. So incorporating caregiver assistance or tech-support services can significantly improve their engagement with eHealth tools.

Nursing informatics is pivotal in enhancing wellness for older adults. A study by Demiris et al. (2013) demonstrates how technology, such as telehealth for monitoring vital signs and cognitive assessments, can provide a comprehensive view of elderly patients’ health in assisted living facilities. This approach integrates data from multiple sources, allowing for early detection of trends and health issues, and facilitating personalized care.

Despite these advancements, older adults often struggle with new technologies due to issues like memory loss or unfamiliarity among others. Lyons et al. (2019) also pointed out that while innovations in telehealth and smart home technologies offer significant benefits, barriers such as affordability and lack of access remain. Needless to say, bridging the digital divide is the most essential. Nursing informatics must focus on making technology user-friendly and offering customized training to improve adoption among elderly patients. Moreover, informatics professionals should advocate for policies and programs that provide necessary resources and support to ensure equitable access to technological advancements.

By addressing these issues, nursing informatics can greatly enhance the quality of care for geriatric patients, improving their overall wellness and effectively managing the challenges of aging.

Indeed, in the age of advancing technology, it is important to note that older patients lack access to and knowledge of the modern healthcare technologies available. You have shared that nursing informatics should create user-friendly medical technologies. I want to add that, while the affordability of devices is a crucial aspect of inviting older adults to access healthcare technologies, I would like to emphasize the importance of easy-to-use and well-thought-out software in the successful inclusion of older adults in accessing advancements in healthcare. I think that companies should aim to simplify the way they delivery their healthcare products in a manner that is not intimidating to users.

I think that a community that needs to be focused on are Persons With Disabilities (PWD). This community is in great importance because they have a variety of needs that need specialized type of care for each disability. A huge challenge that may arise for some PWDs is ineffective communication methods. Those who have trouble speaking, seeing, and other disabilities may face challenges. Nursing informatics should evolve in accommodating PWDS such as by developing screen reader systems for the blind, or video interpretation services for those with hearing impairment.

A specific group that I would like to focus on is the older population, especially those in the community. During our clinical duties, we were introduced to the online appointment system of the Manila Health Department. I have personally witnessed other people who have no access to technological advancements, such as gadgets, the internet, and other resources. With this, despite being an efficient appointment system, some are not able to utilize it and maximize the different services that they offer.

The different factors affecting the application of nursing informatics in this scenario include accessibility of the services, knowledge of how to use them, and sufficient information on the services provided. To address these at our level, we should not stop advocating for health promotion and education to ensure that services would reach all types of people. Through this, we can also help them understand the significance of caring for their health, and in case needed, referrals can also be made. In the bigger picture, it is also crucial to recognize the role of the government in ensuring that these services are widely accessible, not only available, to make sure that programs and services are maximized.

Equal healthcare access to all is the goal of our society, however indgenous peoples have been put in especially vulnerable situations where they are more susceptible to the effects of natural disasters, pandemics, and outbreaks. Their unique language systems, remote geographical locations, and cultural beliefs often serve as barriers whenever they try availing services. With this, a deeper understanding of transcultural factors in nursing needs to be applied in order to avoid our healthcare systems being urban-centric. This problem also goes beyond the issues with our healthcare system. Quality education needs to be far more accessible to everyone to promote unifying concepts upholding the health of everyone.

Adolescents of lower income groups without access to sexual health education experience disparities in their health as compared to groups with more knowledge in that area. Some factors that influence application of health informatics according to De Leeuw et al., in 2020 include:

(1) experience with digital working
(2) perception and meaning
(3) barriers
(4) facilitators
(5) future perspectives

Nurses provide a unique experience to adolescent patients. They are able to mitigate the factors that prevent them from receiving optimum health information and practice. These adolescents come from upbringings with less access to online sources, time spent supporting their family instead of attending school or using public libraries, or fear of approaching health professionals due to potential fees or judgement.

As such, nurses, especially those who practice in depressed or lower income communities, must know how to use their knowledge of reproductive health in order to cater to the adolescent community they work with. By doing so, health disparities can be avoided. They can limit the spread of STIs, pregnancy, and sexual abuse by simply supporting the knowledge gaps in these communities.

As someone who has gone through adolescence, it was an overwhelming experience and challenging life stage. It felt like everything was heightened with the rapid changes in my body, my mindset, and my health. In line with this, there are a lot of adolescents who struggle and are unable to cope with the fast transition. I believe that nursing informatics can help alleviate these struggles through the proper delivery of healthcare services.

Adolescents should be able to have access to online resources that address their needs. During this stage, several factors affect their development, most especially in their sexual health. One factor that influences the application of nursing informatics is the accessibility and literacy of people. It has been a taboo here in the Philippines to talk about sexuality and reproduction. I believe that through the emerging nursing informatics information and communication technology, many adolescents would educate themselves without the need to get embarrassed. This dissemination of information should be in layman’s terms, most preferably in the Filipino language, and without biases to educate these adolescents on how they can take care of their sexual health.

Another factor is their privacy and confidentiality of the data asked. It should be emphasized with nursing informatics that online resources do not, in any way, breach the patient’s confidentiality, and this also increases and builds their trust in online healthcare services.

These are just some of the factors that affect adolescents. I believe that it should also be addressed depending on the patient’s situation and culture regarding their health concerns.

However, due to this conservativeness, adolescents have been engaging in more risky behaviors, thus leading to more negative implications and consequences. Thus, nursing informatics really plays a huge role in raising awareness and educating adolescents and their guardians about what they can do to be more sexually safe and healthy.

One specific group that I think needs the most help in our community is the adolescents, specifically those who are facing teenage pregnancy. I believe one contributing factor to the increased rate of teenage pregnancy in our community is the lack of knowledge and awareness about reproductive health, safe sex, and contraceptive usage. I came to this conclusion because even I was not aware of the availability of contraceptives in health centers until I went to community duties, since it was not properly introduced to the community. It would be beneficial if these matters were greatly prioritized and introduced to the youth to better take care of their reproductive health, and also as an avenue for preventing unwanted pregnancies to stop the cycle and protect them from further complications brought about by early exposure.

With that, I believe that nursing informatics have an important role in reiterating the importance of the aforementioned topics, like the information about complications of early engaging into sex and teenage pregnancies, as well as the availability of contraceptives and health services that one may opt to avail in the nearest health centers. By doing this, it would close the gap between the community and health, which then addresses the root cause of the problem. Moreover, NI could also help to make the information needed more accessible to adolescents, for them to explore more options that is conducive in terms of physical, financial, and social aspects.

Delivering care to older age groups through the application of nursing informatics is affected by several factors. For instance, Heart and Kalderon (2013) emphasized that age is negatively related to the use of information and communication technology due to age-related changes, declines in sensory and cognitive abilities, and difficulties in obtaining technical support. Moreover, it has been reported that older adults tend to adopt technologies selectively based on their perceived usefulness for specific needs.

However, in the context of the Philippines, a significant factor is the lack of accessibility to such platforms. For instance, people in the community may not be aware of the Manila Health Department online portal and may lack the means (e.g., cellular data, compatible phones) to access it. Despite their willingness to seek medical care, they simply do not have the capacity to do so.

Therefore, it is crucial to address these gaps by not only providing the necessary resources but also ensuring that policies and programs offer the knowledge, skills, and attitudes required for both older adults and the general population to effectively adopt and use these technologies. This approach will help make the technology accessible and user-friendly for all.

Deaf clients often experience frustration and mistrust during healthcare encounters due to significant communication barriers (Kuenburg, Fellinger P., & Fellinger, J., 2015). Although digital platforms can assist in helping deaf clients understand medical conditions and treatments, a major factor that exacerbates the gap between them and healthcare professionals is the absence of real-time sign language or closed-captioning services. To address this, nursing informatics must be developed to bridge this communication gap by integrating these services during clinical/community visits, thus improving the delivery of healthcare to deaf clients and ensuring they receive high-quality equitable care.

This is an eye-opening answer, Alessandra! Health care services should be received by all people and they should not be limited to those who have the physical capacity to avail them. Persons with disability are also a vulnerable group of people and health care must be more accessible because of their special needs. Hence, I very much agree with you that the communication gap must be addressed by integrating various services not only in hospitals but also in communities to improve the delivery of healthcare services to PWDs and ensure equitable nursing care.

Wow, Rein! I really liked the idea of integrating real-time sign language and closed-captioning into our healthcare system delivery. I feel like minority groups, such as people who are hard of hearing, do not get the attention and service they need in our healthcare system. I do hope that, in the near future, we become one of the change agents to improve their situation and give them the treatment they deserve. Additionally, I also think that integrating real-time sign language not only in the news but also in all media platforms and healthcare services would allow people who are hard of hearing to empower themselves, as well.

Professor Baccay highlighted during our lecture on Nursing Care of High-Risk Mothers that there has been a 35% increase in pregnancy among children below the age of 15 in 2022. This is very alarming. Given that two years have already passed, this data may have increased. And so, I would like to focus on young adolescents, both male and female, of reproductive age.

My interest in this issue stems from my community experience last semester; wherein, I had two clients who mentioned that they became parents at a young age, but still have reservations with the use of family planning. More importantly, I remember the male client wanting to expand his family, while the female client expresses otherwise.

One factor that influenced the application of nursing informatics in delivering healthcare services to my client was their inadequate knowledge of their options. Most of the time, they rely on information from the internet, without knowing whether the website is credible or not, and if the information they obtain is factual or misleading. Their awareness of services they can avail of is also quite insufficient, especially since they are also not well-versed in the resources of their nearest health center.

In this sense, reproductive health and family planning must be advocated. It can be done through a comprehensive education, which includes introducing and offering the available options to increase their health literacy and allow them to make informed decisions and empower them to maximize their resources according to their needs.

Very well said, Jess! The alarming rise in teenage pregnancy cases truly is something that we nursing students should be involved and concerned to. Although, I would like to add in your mentioned factors the idea of poverty and lack of resources. We learned that health is an all encompassing discipline which is influenced by a lot of factors, socioeconomic to be included. By addressing this gap, we can also achieve better outcomes in preventing the furtherance of this issue in our society.

I agree with you, Jess! During our N13 rotations, I also have handled two teenage pregnancy cases, aged 16 and 17 who are still enrolled at their respective high schools. It is indeed very alarming that teenage pregnancy rate is increasing in our country, and the age of the mothers is decreasing. By advocating for reproductive health education and family planning, these rates can be reduced and the overall well-being of young women can be improved. Providing access to proper reproductive health education in schools and communities will empower teens to make informed decisions about their bodies and futures. Additionally, we, as future nurses, can play a critical role in educating not just the youth but also their families about the importance of these topics.

A priority healthcare need for pregnant women is ensuring continuous access to comprehensive prenatal care, especially in underserved areas where healthcare resources may be limited. Utilizing nursing informatics, through telehealth and mobile health (mHealth) applications, enables real-time monitoring of maternal health, such as tracking vital signs, fetal movement, and overall well-being. Technology-related factors also play a crucial role in the application of nursing informatics in the delivery of healthcare services to pregnant women. The perceived usefulness and ease of use of health information systems play a crucial role in their adoption. If the systems are seen as improving workflow, reducing errors, or facilitating patient care, nurses are more likely to use them.

However, it's important to acknowledge that several challenges exist in the successful application of these informatics tools to ensure equitable care for pregnant women. Factors such as limited access to technology, disparities in digital literacy, and concerns regarding data privacy and security may hinder the widespread adoption of telehealth and mHealth applications in prenatal care delivery. Overcoming these barriers requires concerted efforts from healthcare organizations, policymakers, and technology providers to ensure that pregnant women from all backgrounds have equal access to these essential resources.

In conclusion, it is imperative to recognize the critical importance of prenatal care for expectant mothers. Ensuring continuous and equitable access to prenatal care services, supported by nursing informatics, is essential for promoting healthy pregnancies, reducing maternal and infant mortality rates, and ultimately enhancing the well-being of communities. By addressing the existing challenges and leveraging the potential of nursing informatics, we can work towards achieving comprehensive and inclusive prenatal care for all pregnant women, regardless of their geographical location or socioeconomic status.

A priority healthcare need of my community is to have increased physical activity since most of the people in my community are sedentary and are at risk for NCDs. There are also no places in my community for public physical activity. The sports complex is in another barangay. To address this issue, nursing informatics must be applied, however it faces the following barriers:

Inadequate education of the population - The people in my community do not prioritize physical activity because they are not aware of its benefits. Some of them may not even see their sedentary lifestyle as a problem. Their level of education may also hinder them from understanding how to use technology.
Inaccessibility to technology - In my community, many do not have access to electronic devices, mobile data, and Wifi, as such, many also do not know how to use these technologies and have no knowledge about how to communicate their grievances to the officials. Although some may know of the importance of exercise and want to incorporate it in their lifestyle, due to inaccessibility of technology, they would not know how to start (e.g. start following youtube videos instead of waiting for government programs alone).
Economic restraints - Some may have access to the internet and can research nearby places/videos to exercise, but due to economic constraints, they would rather consume their internet for their or their children’s school requirements and stay at home to lessen expenses.
Neglect of those who are in the middle class - Those who have the privileges in my community are apathetic towards the needs of the less fortunate, They know the importance of exercise, practice it themselves, can allow others to join them in exercising, and can communicate the issue to the officials through social media and through other means, however, they choose to remain passive because they are not affected by the issue.

Adolescents experiencing teenage pregnancy face higher risks of maternal and neonatal complications during and after childbirth (Fernandes, 2015). Additionally, many of these young mothers come from socioeconomically disadvantaged backgrounds and have low health literacy due to limited access to resources. This can create barriers to receiving proper prenatal care, health education, and support. By addressing these factors through nursing informatics we can lower the maternal and neonatal mortality rate in the Philippines by improving access to health information, enhancing health literacy through age-appropriate content to ensure comprehension, and assuring confidentiality to enhance patient participation and reduce anxiety.

Thank you for bringing this up. I completely agree with this, Coco. Teenage pregnancy poses serious risks, and many young mothers lack the resources and health literacy they need. Nursing informatics can make a huge difference by improving access to relevant health information and ensuring it's presented in a way adolescents can understand. Enhancing confidentiality also plays a key role in encouraging young mothers to seek care without fear or anxiety. By addressing these issues, we can improve maternal and neonatal outcomes and help lower mortality rates in the Philippines.

For women past reproductive age, managing menopause and aging-related health issues like osteoporosis, cardiovascular disease, diabetes, and hypertension is crucial. Older women often face challenges with health literacy, affecting their understanding of these conditions and preventive care. Some may find it difficult to comprehend and act on health information, which can hinder their ability to manage their conditions effectively. Many older women may have limited experience with digital technology, making it challenging for them to navigate electronic health records (EHRs) or health management apps. Even if they can use technology, older women may struggle with understanding and interpreting health information presented through these systems due to varying levels of health literacy. Nursing informatics can address this by providing accessible educational resources and user-friendly tools. These informatics systems can offer clear information, personalized health plans, and reminders, helping women manage their health conditions more effectively and improving their overall care.

Let me share a similar experience in our Public Health Nursing I, wherein some of them approached me to ask questions regarding the MHD appointment system and vital signs taking. They said that they do not understand navigating through the technology, which delays their consultations in the health centers. With this, I inferred that it is important to exemplify our calls for action to enhance the health education systems in the country. Hopefully, these problems will be resolved soon, through policy changes and development.

During community duties, it is evident that the adolescent group is one of the vulnerable populations. Being someone who has gone through this life stage, it tends to be confusing and overwhelming, especially when there is only little to no guidance. Factors that influence the application of nursing informatics may include social determinants of health, such as the access to adolescent health information and services.

Herewith, one significant application is the availability and access to online resources. Especially focusing on their sexual health, sex education is a crucial aspect in preventing the rise of the cases of teenage pregnancy (Yamat et al., 2023). It is undeniable that the Philippines is already implementing policies regarding this, with the Reproductive Health Law, but nursing informatics can help further in information dissemination. Personally, I did not know that contraceptives are free and available at health centers, which show the gap in education.

Delving deeper into this, there is a lack of access to such useful information that can mitigate the above problem. Upon observation, a lot of people in the community do not know and understand how to use the Manila Health Department website and how to book for appointments, and lack the data and/or internet to access it.

With this, nursing informatics is essential in addressing these barriers. Health literacy and education, as well as access and availability of information to the general public must be prioritized. As such, social media platforms, especially Facebook and TikTok, that are heavily used by adolescents may be utilized. Consequently, policies and programs that may address the gaps in adolescent health, and the whole population, should be developed and implemented.

In Kamali et al.’s (2018) study, several factors were highlighted that influence the application of nursing informatics in delivering healthcare services to pregnant women. One significant factor is the varied information needs of expectant mothers. Pregnant women often seek detailed information on pregnancy, childbirth, and postnatal care, including topics such as fetal development, nutrition, and labor pain management. This diverse demand underscores the importance of tailoring information resources to meet specific needs, which can be facilitated through effective use of nursing informatics tools.

Barriers to information access also play a crucial role. Challenges such as lack of knowledge, financial constraints, and language barriers affect the ability of women to obtain relevant information. For instance, many scientific resources are available primarily in English, which can be a significant obstacle for non-English-speaking populations. Addressing these barriers through accessible and multilingual informatics solutions can improve information dissemination.

Additionally, sources of information significantly impact how pregnant women receive and use health information. Physicians are a primary source, but the Internet and family/friends also play important roles. The increasing reliance on digital platforms highlights the need for informatics systems that integrate these diverse sources, ensuring comprehensive and timely information delivery.

Finally, cultural and regional factors affect information needs and preferences. For example, the study notes differences in information-seeking behavior among women based on their geographical location and education level. Informatics systems must be adaptable to these variations, providing relevant content and support that aligns with local practices and expectations.

In summary, nursing informaticists must consider these factors—information needs, barriers, sources, and cultural differences—to design and implement effective health information systems for pregnant women. This approach ensures that the technology supports comprehensive, accessible, and contextually appropriate healthcare services.

The incidents of mental health illnesses have rapidly increased over the past few years. Many lives have been affected. However, I believe through nursing informatics, many mental health illness patients can be reached and given care, but some factors hinder the acceptance of care to these patients.

One of which is the limited access to mental health services. Nursing informatics can be incorporated to reach more patients, such as teleconsultations. Also, there is still a stigma surrounding mental health, which prevents patients to seek help. Thus, nursing informaticists play a role in raising awareness about mental health to provide awareness and education surrounding the issue. Additionally, given we are in a digital age, data privacy and security is also a concern among mental health patients. They are scared to seek help since they are scared that other people will judge them for seeking help. Thus, nursing informatics helps address this by providing additional security measures.

As someone who is enthusiastic with toddlers, I want to prioritize this age group in the provision of healthcare and nursing services. Oftentimes, this specific stage in life and development is overlooked as the stage where children are just naughty, active, and difficult to handle especially in acts of promulgating healthcare outcomes and services. Nevertheless, I see that although these sentiments are true, toddlers require attention and understanding as they are in the formative stage of their psychosocial development.

One factor that I think nursing informatics can and will be useful in addressing toddler’s nursing care needs is the responsible use of technology. As mentioned earlier, this stage is the formative years of the child wherein exposure to technology is very much likely to happen. They are explorative and curious of the world around them and having access to technology such as smartphones, tablets, television, and even toys are inevitable. By responsibly teaching and incorporating technology with the needs of toddlers, nurses can ensure a more holistic approach that would implore compliance and participation from the toddlers as they perform their nursing interventions.

Another factor would be the integration of age-appropriate activities and paraptions that are results of responsible, extensive application of nursing informatics as a field of study and practice. The creation of engaging forms of play and activities not only ensure cooperation but ensures an avenue of connection, communication, and engagement with toddlers as nursing interventions are performed with them. Moreso, the recognition of differences in each toddler’s healthcare needs is also considered in this as we know that every child has their own needs, wants, and preferences.

A priority healthcare need of my community (Cavite) would probably cater around persons with substance use disorder (SUD) Through access to voluntary, community-based rehabilitation services rather than compulsory or punitive treatment. The application of nursing informatics in this context is influenced by several factors.

First, secure data collection and integration are crucial to ensure individualized care, but users may be reluctant to share sensitive information due to fear of legal repercussions. Nursing informaticists must prioritize data confidentiality to protect patient rights.

Second, stigma and ethical concerns around compulsory rehabilitation challenge the principle of patient autonomy. Informatics tools, such as patient portals, can empower users by allowing them more control over their treatment, aligning with a human rights-based approach. Third, resource allocation can be optimized through nursing informatics by using real-time data to monitor patient progress and distribute care resources efficiently, such as counseling or psychiatric services. However, limited resources in rehabilitation centers may hinder the full potential of these tools.

Lastly, informatics facilitates collaboration between healthcare providers, enabling better coordination of services across various stakeholders, including medical, psychiatric, and social support teams. These factors highlight the potential of nursing informatics to improve the delivery of healthcare services for drug users by promoting patient-centered care, optimizing resources, and enhancing ethical treatment approaches.

One of the most significant healthcare issues that I have observed in our community is the prevalence of malnutrition, particularly in school-aged children, especially those who are no longer supported by the supplementary feeding program. Accordingly, the factors that influence the application of nursing informatics in the delivery of health care services would include (a) efficient data collection, (b) personalized care plans, (c) education and engagement, and (d) outcome evaluation.

First and foremost, monitoring malnutrition in young individuals by assessing the group's growth parameters, dietary intake, and nutritional status over time is significant. Nursing informatics may coordinate with the Barangay Health Centers and utilize tools such as Electronic Health Records (EHR) to collect and track growth measurements, dietary habits, and nutritional deficiencies, enabling regular monitoring and early identification of malnutrition trends.

Second, using the collected data, nursing informatics may construct a care plan corresponding to the collected data with appropriate interventions to manage malnutrition in the community. Clinical Decision Support Systems (CDSS) can assist in creating and adjusting personalized nutritional plans. These systems can recommend tailored interventions and track their effectiveness by integrating data from dietary assessments, growth charts, and medical history. Furthermore, it is significant to stress the need for collaboration with families and the barangays and consider other factors such as the socioeconomic status of the affected group, so that the intervention may be appropriate and attainable.

Third, it is significant that we educate the parents or the primary caregiver regarding this concern. However, it may be complex as managing their child's diet may coincide with cultural and social hurdles. Therefore, mobile health apps and patient portals of informatic technologies may be utilized to offer educational materials and dietary guidance. These applications can also help healthcare practitioners and families communicate more effectively, providing recommendations and measuring progress.

Lastly, the proposal holds the potential to significantly improve the nutritional status of the community. Nursing informaticists must assess the effectiveness of nutritional interventions, and tracking improvements in nutritional status requires robust data analysis to acknowledge the need for further adjustments. Accordingly, informatics technologies can be used to examine data on treatment outcomes, such as growth measurements, nutritional consumption, and overall health. This aids in determining the efficacy of therapies and directing future care methods.

One of the priority healthcare needs of the geriatric population, or older people, is to maintain their quality of life by preventing complications and managing the diseases they may have. Older people are more likely to develop chronic illnesses as their bodies undergo the wear and tear of organ systems over the years. Most of them are also unable to visit the hospitals or health centers to communicate their cases with healthcare professionals due to bodily restrictions and others. This made their population group, one of the main targets of the growing telemedicine. With advancements in technology and informatics in nursing practice, much of the geriatric population struggles to keep up with these changes. In the five categories of adopters in the context of technological innovation adoption, they are often considered either part of the late majority, who are skeptical of new changes, or the laggards, who are set in their ways and believe that technology hinders healthcare operations. Older adults have limited familiarity and exposure to technology, making it challenging for them to utilize digital health tools effectively. Furthermore, not all have the capacity to access such technology, limiting their ability to engage in telemedicine for check-ups. Lastly, many are reluctant to use technology due to concerns about the security of their health information on digital platforms.

I once worked with a blind community in my province for our senior high school research. During our discussion with the group, they were able to identify several healthcare needs, including the need for a thorough check-up, especially since their health problems are often overlooked or overdiagnosed by healthcare professionals (Dovidio & Fiske, 2012; McColl et al., 2008 as cited in Heydarian et al., 2021). Their condition also hinders them from accessing healthcare services since most healthcare facilities are far from their areas, and some of them do not have companions who will assist them during their travel and relay doctors' orders, leaving them more vulnerable to different illnesses.

Their situation necessitates the application of nursing informatics to improve the delivery of healthcare services to this group. Most blind people are now adept at using the different features of their phones for communication. With the invention of the text-to-speech feature, they can instantly listen and reply to messages. They can even browse through social media platforms and connect with their loved ones from different places. With this, nursing informaticists may jumpstart the creation of a teleconsult wherein dialing a certain number will allow this community to avail healthcare services and have their medicines delivered to them, especially those far from healthcare facilities. They may even jumpstart the creation of healthcare materials or medicines with the Braille alphabet inscribed to ensure that the community is taking the right medicine. We can also apply nursing informatics to create prescriptions and medical records using the Braille alphabet so that the community will have the right to take control of their health. We can use nursing informatics to create a more inclusive healthcare system that empowers everyone.

From my observations, elderly individuals who lack access to technology and information about over-the-counter (OTC) medications and antibiotic resistance face critical healthcare needs that are often overlooked. Despite advancements in technology, many older adults struggle to keep up and are unaware of the risks associated with self-medicating and improper antibiotic use.
Even when technology is available, older adults may not have the skills or confidence to use digital tools effectively. This limits their ability to access and understand important information about medications and health guidelines. Limited health literacy further complicates their understanding of OTC drugs and the importance of proper antibiotic use, including potential drug interactions.
Addressing these issues through targeted interventions can improve nursing informatics and enhance healthcare for elderly individuals, particularly in managing OTC medications and understanding antibiotic resistance.

Nursing informatics, the integration of nursing science and information technology, plays a crucial role in delivering healthcare services to pediatric patients. However, several factors influence its successful application, particularly parental involvement and data privacy.

Parents or guardians often serve as the primary caregivers for children, making their involvement essential in the use of nursing informatics tools. These tools must be designed to be user-friendly and provide clear guidance to support parents in managing their child's health. By offering easy-to-use interfaces and informative content, informatics systems can empower parents to monitor symptoms, ensure medication adherence, and make informed lifestyle choices.

Data privacy and security are paramount in pediatric healthcare due to the sensitive nature of children's health information. Informatics systems must comply with relevant regulations to protect the privacy of this data. This includes implementing robust security measures to prevent unauthorized access and ensuring that information is only shared with authorized healthcare providers.

In conclusion, the successful application of nursing informatics in pediatric care requires careful consideration of parental involvement and data privacy. By designing user-friendly tools and prioritizing data security, healthcare providers can leverage informatics to enhance the quality of care for pediatric patients and support parents in their role as caregivers.

A priority healthcare that I have noticed in my community is the lack of knowledge in sex education for adolescents. It was mentioned during one of our discussions in maternal and child nursing that teenage pregnancy remains prevalent in today’s world because talking about sex within the family is considered awkward for the family members when in fact sex education should be introduced to children as early as when they are knowledgeable enough to understand what are genitals and how it should not be touched by others aside from themselves. Moreover, since the Philippines is a religious country, sex is a taboo topic that is avoided by a lot because it can be considered as sinful to be talked about.

It is important to talk about sex education in both the household and the school because it brings about a new topic of accessibility. By educating students about free access to contraceptives and free social clinic services, they are able to maintain a positive outlook on sex, prevent teenage pregnancy, as well as prevent acquiring sexually transmitted disease (STD).

Considering that mental health is a very sensitive and serious topic, not all are comfortable enough to share their vulnerable side with another person– a stranger at that. With the help of nursing informatics, telecommunication can occur where both parties do not have to see each other, making it easier for the patient to open up to the doctor. To add, since mental health is still kind of a taboo topic (especially for older generations), the anonymity of it prevents the occurrence of being rumored about in clinics (other patients giving out judging looks to the patient, giving side remarks, etc.). This then gives patients a little more courage in actually reaching out their hands and asking for help from professionals.

Good point, Alyssa! The anonymity feature of TeleHealth consultations is indeed helpful in making patients feel more at ease in expressing their concerns, especially since mental health is often stigmatized. In particular, men exhibit higher rates of suicidality because they are more likely to be associated with the machismo culture, which discourages them from getting mental health treatment. Thus, through this approach, their anonymity is safeguarded, which can inspire more individuals to seek therapy while also fostering a supportive environment for talking about mental health concerns.

Hi Alyssa! I'd like to commend you for pointing out one of the advantages of nursing informatics in telecommunications: anonymity. In a world where people are constantly ostracized for their circumstances, it is saddening to see that they are left to suffer alone since it is what they think would help them the most. With the help of telecommunications, people can be heard and receive health without biases, improving their health-seeking behavior in the long run since they are now more confident to express their health concerns.

Older adults are a specific group of people who exhibit vulnerability because of possible multimorbidity, functional incapacity, and psychosocial problems (Beales & Tulloch, 2013). The application of nursing informatics in delivering healthcare services to them could either hinder or promote health outcomes.

In our country, according to the reports of the Philippine Institute for Development Studies, Filipinos aged 65 have the lowest digital literacy levels among other age groups (Ernest Cu Urges Seniors to Go Digital, n.d.). This factor may affect the positive health outcomes of health care delivery for geriatric patients as some of them may not be comfortable or proficient in utilizing these modern technologies.

Hence, this factor associated with geriatric patients may influence in optimizing technology that would be appropriate for older adults, towards promoting their health outcomes (DeAngelis, n.d.).

On the other hand, nursing informatics plays a significant role in delivering health care services to geriatric patients. As part of normal human physicological development, age-related forgetfulness is normal in older adults because of functional deterioration that may occur in their age group (National Institute for Aging, 2023). Hence, digitally storing their data such as in electronic health records, may provide an overview of patient information, history, and other pertinent data needed in order to provide patient-centered care to them.

In this aspect, the factor of age-related forgetfulness could influence the nursing informatics application to put an emphasis on the records obtained from older patients in order to track and maintain the accuracy of care given to them.

As HIV continues to become a global health issue (WHO, 2024), Persons Living with HIV (PLHIV) also continue to become a priority of healthcare. With nursing informatics, there are currently various tools that PLHIV can access for their treatment regime. For example, EHRs help document their information to track their history. Teleconsult services allow them to access remote consultations, and data analytics may help in analyzing current patterns that allows for better management of their care. However, these nursing informatics tools are influenced by various factors that influence the delivery of healthcare services to PLHIV.

Firstly, cultural stigma surrounding PLHIV prevents them from accessing healthcare services to avoid disclosing their status. This influences the use of EHRs and dana analytics that need their information in documenting pertinent information or generating patterns to address the HIV/AIDS epidemic (Turan et al., 2016).

Secondly, it’s saddening to know the reality that discrimination may also come from the healthcare providers that also influence the application of nursing informatics tools in the delivery of healthcare services, as they create an unpleasant environment for PLHIV to engage in care (Zarei et al. 2014).

Thirdly, the access to healthcare delivery services also influences the application of nursing informatics tools relying on patient information, as PLHIV may have a difficult time in knowing where to seek appropriate care. Fortunately, In the Philippines, there has been increasing progress in the treatment and prevention of HIV with the enacted lPhilippine HIV and AIDS Policy Act of 2018 (Gangcuangco & Eustaquio, 2023).

With this, it is high time to further strengthen our advocacies towards the betterment of healthcare for PLHIV.

The Philippine General Hospital Outpatient Department (PGH-OPD) uses the Online Consultation Request and Appointment (OCRA) System for scheduling appointments with physicians. To use OCRA, patients must first create an account by providing their personal details, setting a password, and supplying an email address. After account creation, they can request an appointment. However, appointments can only be made on weekdays between 6:00 AM and 6:00 PM, and the OPD limits the patients seen each day to 500 individuals. During our assignments at the OPD, I observed the high volume of patients, often exceeding 500 per day, coming from various provinces.

For geriatric patients, regular consultations with their physicians are crucial as they are prone to age-related diseases. However, this system poses challenges for them, especially since many are not tech-savvy and may have impaired vision. I recall assisting a geriatric patient who struggled with OCRA. She arrived without an appointment and had difficulty navigating the system due to a lack of email and poor memory regarding her password. Additionally, older patients often face difficulties with their devices due to a decline in sensorimotor skills.

Given these challenges, I believe we should consider more flexible approaches for geriatric patients who seek medical care without a prior appointment. Allowing them to be seen despite not having a scheduled appointment would acknowledge their efforts and time spent traveling to the hospital, and prevent them from leaving without receiving the necessary consultation.

Thank you for pointing this out, Gia! I definitely share the same sentiments as you do because I have personally experienced this struggle with older adults in the OPD-OBYGN Department during one of our N13 rotations. While it is truly a great feat to be able to utilize the convenience provided by modern technology, it has been a challenging circumstance for our older adults to access the healthcare services that they wish to avail, making it a lot more heavier on their end to receive the care that they need. Thorough evaluation of the systems to accommodate different age groups must also be explored and expanded further to ensure that each and everyone shall have equal access to healthcare.

I believe that one priority healthcare need that I was able to ascertain, back when I was doing my community rotation, would be the health-seeking behavior of pregnant women, especially those who are not too keen on making sure that they can accomplish the required number of prenatal visits. However, it is also important to realize that many people in the community do not necessarily have the resources and technology that we have easy access to, hence, the delivery of needed services may not be as easy and as black and white.

With that, the following are the factors that may influence the application of nursing informatics in the delivery of healthcare services in the specified group above:

Digital Literacy: Some patients may lack the technical skills that are needed to navigate new digital tools effectively (especially those who never had proper access to the advent of technology); this is why we nurses need to assist them in navigating said tools.

Access to Technology: Many individuals in the community lack or may have limited access to gadgets such as smartphones, computers, and internet connections, all of which are needed to access different crucial resources online.

Stigma and Lack of Trust: Concerns about the security and privacy of personal health data may prevent women from using digital tools.

The group I would choose would be adolescents. Being in their puberty stage, many adolescents may want to explore things such as their sexual identity, relationships, and overall sexual health. However, as a person living in Caloocan, I know how very limited sexual education is in our community. This lack of comprehensive sexual education often results in misinformation and misconceptions, which contributes to a high rate of teenage pregnancies. Without proper guidance and resources, adolescents may not fully understand safe practices, consent, and the emotional aspects of relationships, leading to unintended consequences and significant challenges for both the individuals and their communities.

The importance of prenatal care and education has been emphasized repeatedly in our previous nursing courses involving maternal and child health. The health of both mother and child throughout and after the pregnancy is greatly affected by the attainment of quality prenatal care as it allows for monitoring of their health, provision of essential supplements for the baby’s development, and helps the mother identify signs and symptoms of possible complications indicating the need for consultation. Because of how vital it is for pregnant women to have access to prenatal care, nursing informatics should be properly utilized to accomplish improved availability amongst the community.

Nursing informatics contributes to the creation of technologies that can improve accessibility to prenatal care services, which can in turn improve the adherence of pregnant women to necessary prenatal check-ups. However, a factor affecting the use of informatics in prenatal care could be the availability and accessibility of current technology marrying the two, especially in the Philippines. In some countries, there are developments being made towards creating applications for accessible maternity services (Dorst, Anders, Chennupati, Chen, & Purcell, 2019), displaying information such as test results, scheduling and making reminders for regular check-ups, and making contact with healthcare providers easier. Currently, there may not be many applications or websites available for easier access to prenatal care, but locally, many nurses do create online material such as videos and podcasts that involve prenatal care and vital information for pregnant women–so while there may still be some way to go to fully incorporate nursing informatics and technology with prenatal care, some steps have been made locally to make it happen.

It has long been established that aging and health are correlated. With the gradual decrease in their physical and mental capacity, the elderly have an increasing demand for care due to several risk factors (World Health Organization, 2022). Moreover, those who live in indigenous communities lack access to health as they cannot be reached easily by healthcare professionals. A study conducted in SoCSarGen affirmed that the elderly in indigenous people communities need medical services at home (Department of Science and Technology - Philippine Council for Health Research and Development, n.d.).

Everyone should have equal access to healthcare and as such, the elderly in these communities should have access to culturally sensitive, preventive, and disease management services. Some of the factors that influence the application of nursing informatics in delivering healthcare services to them are access to technology and language barriers. Those living in remote areas may have limited access to power, internet, or gadgets that may hinder them from availing of telehealth services and accessing information. Furthermore, they may be fluent only in their indigenous language. Rather than viewing them as simply laggards, we should advocate for better solutions and call for the government's attention to invest in technology and healthcare facilities in these communities. Moreover, they should also ensure community partnership. In this way, we can fully maximize the benefits of nursing informatics, ultimately leading to improved overall health and well-being of the elderly indigenous people.

Hypertension being one of the top causes of death in the Philippines, and with its steady increase from 22 percent in 1993 to 25.15 percent in 2013 (Rosa et al., 2024), shows that there are gaps or problems in the provision of care for the patients. This being widely prominent in communities with lower-income Filipinos, it becomes a struggle to apply nursing informatics to this target group. Rosa et al. (2024) has identified that “almost half of hypertensive Filipinos are still not aware of their condition, and only 27 % have it under control.” Which means that the dissemination of information is one of the influential aspects in the actions taken by individuals that have hypertension. This is where nursing informatics should be applied, but then again, as I have said, this problem is common in low income areas, which becomes a challenge when technology and tools are not readily available, and what is available are traditional pen and paper tools that are inefficient both in inputting data and spreading information amongst the community. With lack of tools or funds, even if the information is there, it could not reach the target population, leading them to not be aware of their condition, or their risk of having it, or the symptoms that it may cause, which means no action will be taken. This is detrimental because the decision to go health centers and have health seeking behavior lies on the individual persons, and without that information and awareness, it is likely that they would not seek and receive the care that they need.

Hi, Christian! I completely agree with your insight on the challenges of managing hypertension in low-income communities. The statistics you mentioned are alarming and I have also encountered hypertensive patients in the community who perceive hypertension as not urgent, causing them to stop taking their medications, particularly due to the cost of maintenance medicines. This only exacerbates the issue. Nursing informatics could play a key role in addressing the gaps but as you pointed out, the lack of access to technology is a major barrier. Digital tools could actually help raise awareness about hypertension, its risks, and potential complications which could improve medication adherence and health-seeking behavior. Integrating technology into community health efforts can bridge gap in care but it will require collaborative efforts and investment both in infrastructure and education to ensure accessibility for everyone.

During the pandemic, telemedicine emerged as a groundbreaking approach to healthcare delivery. While highly beneficial, it presents limitations for vulnerable populations such as the elderly, homeless individuals, and those with inadequate language skills (Haimi, 2023).

Digital literacy is a primary factor hindering the application of nursing informatics in healthcare services delivery. These vulnerable groups often lack the knowledge and skills to navigate telemedicine platforms and participate in virtual consultations, creating a digital divide. Additionally, language barriers can pose significant challenges, particularly in interpreting and communicating healthcare needs effectively.

Furthermore, access to reliable internet connectivity or signal service is essential for effective telecommunication. While healthcare providers did their best to adapt to the situation, training and education can better equip them to adopt new technologies and influence the implementation of nursing informatics.

A priority healthcare need in my community is improving vaccination coverage for adolescents and the elderly. These age groups are particularly vulnerable to diseases such as cervical cancer, pneumonia, influenza, and hepatitis B. These illnesses can be prevented through vaccination as well as reduce both the health risks and their associated medical costs.

For example, in 2019, only 29.6% of people aged 60 and above were aware that flu vaccines were available at health centers. Of those aware of the service, only 36% actually received the vaccine (Cruz et al., 2019). This outcome may be partly due to low publicity on these services.

While people are generally aware that vaccinations are available for infants, awareness is much lower for vaccination services targeting adolescents and the elderly. Social media offers a powerful tool for spreading information quickly and widely. By leveraging social media to raise awareness about vaccination services for all age groups, we can improve vaccination rates and better protect our community's health.

I think one of the most important but often overlooked priority healthcare needs is mental health. In my opinion, I think that one of the main factors that influence the application of nursing informatics in the delivery of healthcare services to individuals with mental health problems is the constant need for patient engagement and monitoring.

There are many existing technological tools that contribute to easier and more efficient patient interaction and monitoring, which include the utilization of electronic health records (EHR), telehealth services, and mobile apps. EHRs provide real-time updates about a patient’s condition, which can help in monitoring mental health changes and possible treatment options. Additionally, these ensure that all members of the healthcare team have access to patient information, which promotes the collaboration of care. They can also help in tracking patient progress over time and identifying patterns that may indicate changes in mental health status. Moreover, telehealth services and mobile apps provide virtual consultations and support, which reduce barriers to accessing mental health services and allow patients to receive timely and continuous care (Alenezi & Alghamdi, 2022).

While technology has many advantages, it is important to also consider another factor that influences the application of nursing informatics in mental health care, which is the balance of technology use with the human aspects of care. Nurses play a significant role in providing emotional support and understanding, which technology cannot fully replicate. Technology should serve as a form of support for human interaction, not a replacement. Effective mental health care requires a combination of technological tools and compassionate personal care.

At present, I'd opt to concentrate on people who have mental illnesses or diseases. While the community has accepted and acknowledged these issues, there remains a stigma and the assumption that mental health should not be prioritized. Some people continue to believe that mental health is not as important as physical health. Several factors continue to influence the use of nursing informatics in healthcare delivery for such populations. There are a few government policies and regulations governing the use of health information technology in mental health treatment facilities. While some institutions provide mental health care, many people have limited access due to financial limitations.

There is also an absence of understanding and ability to use informatics tools and applications to seek the assistance of mental health experts. Some people are also anxious about their privacy and security in terms of mental health, fearing that their condition would be leaked and criticized. Concerns regarding stigma and prejudice influence attitudes toward the use of technology in mental health care. We need to advocate for government policies and specific rules and use informatics tools to increase access to mental health care while addressing financial difficulties. If we focus on these aspects, healthcare organizations and the community can encourage the effective use of nursing informatics in mental health services.

The priority healthcare need of pregnant women is accessibility to quality prenatal care. Factors that may influence the application of nursing informatics in delivery of prenatal care would include access to technology, data privacy, healthcare provider’s training, policies, and cultural factors. For access to technology, a digital divide still exists to this day as not all women are able to access telehealth services, wearable devices, and mobile health applications that can monitor fetal development and provide education on maternal health. Pregnant women are also hesitant in providing personal and medical data due to their concerns on data privacy and security. These tools must ensure that they are compliant with the data regulations to protect the patient’s privacy. Another factor would be the extent of knowledge that healthcare providers have in utilizing technologies and the policies that integrate nursing informatics into routine prenatal care. Restrictive policies and lack of funds act as barriers on women’s accessibility to prenatal care. Lastly, cultural attitudes of women toward pregnancy and technology may influence their acceptance on the use of nursing informatics such as living in a community where pregnancy is valued or seen as a highly private matter may deter them from utilizing digital tools.

In the Philippines, one of the most pressing healthcare needs is mental health services. Despite the increasing awareness of mental health issues, access to quality care, remains a significant challenge. However, nursing informatics has significantly enhanced the delivery of mental health services through digital tolls such as mobile apps, virtual counseling platforms, and other patient portals that can empower individuals with mental health conditions to actively partake in their care. These tools help bridge the gap for individuals who face barriers to traditional in-person care and prefers virtual counseling due to fear of judgement and or stigma. Moreover, patient portals provide individuals with a way to monitor their condition and access their medical records, treatment plans, and communication with healthcare providers. This empowers patients to take a more active role in their mental health care, promoting self-management and informed decision-making. By leveraging these digital tools, mental health services can become more accessible, personalized, and supportive, leading to improved outcomes and patient satisfaction.

Since a significant portion of the country remains unreachable by the developments in technology, the reach of mental health apps and virtual counseling platforms is still limited. Likewise, many individuals, especially older adults and those from marginalized communities, may lack the technical knowledge to use these tools. The use of digital media in healthcare is still evolving in the Philippines. There is still a need for clearer policies and regulations governing telehealth services, patient data privacy, and security.

Last May, when I had my duty at the Icasiano Health Center, I noticed how a lot of older aged people were struggling with technology when it came to setting up a schedule online for a face-to-face consultation. In the health center, an appointment must be scheduled online before having your consultation. Inevitably, there were those who came with no appointments and some of their frustrations was that they did not know how to navigate through their phones nor the website. It took longer for them to be able to avail the healthcare services they were seeking but in the end the healthcare workers was able to assist them and provide their healthcare needs.

A priority healthcare need for individuals suffering from mental disorders would be efficient through nursing informatics with accessible mental health support. Some factors that influence the application of NI in delivering care to people with problems in mental health are the online health services availability, patient data security, and the access to electronic health records. Having the option to consult online is available to those individuals who may face stigma or have no physical access to services. The data privacy and the health records of the patients are also critical as these contain sensitive information, hence, safeguarding these aspects builds patient trust.

Factors that influence the application of nursing informatics in the delivery of healthcare services to geographically isolated areas

Recalling from the study guide, nursing informatics is the utilization of information technologies in the nursing profession. According to Darvish et al. (2014), the elements that should be considered in the implementation of nursing informatics are advanced systems, internet and network, and health promotion.

For the following discussions, I will them on the Philippine context. In 2023, the DICT has reported that olny 35% Filipinos are connected to the internet (Clapano, 2023)! The lack of proper education and resources on internet and network affects the accessibility of healthcare services in general. This inefficient internet connection then leads to lower quality of life. Public and private institutions have increasingly shifted to social media platforms for disseminating information. If there is inaccessibility to such platforms due to internet problems, it can be strong case that those who seek health care do not know what standards of care they should receive given that they are geographically isolated. Moreover, the lack of education on maneuvering and sieving through information online can increase the hesitancies of the target group in approaching assistance from health institutions.

Moving on to the next key element, health promotion seems to be lacking in the Philippines for the “complete” health services are centered on mainland areas and the source of health care in geographically isolated areas are the “faith healers”. The evident inequalities in healthcare services aggravate the degree of isolation brought upon by spatial challenges of those who live far-flung areas. As areas become more distanced from central areas, the extent of healthcare information also dissipates due to factors of healthcare systems and not only due to the distance.

I agree with you, Uoiea! It's great that you have included data in your answer. Indeed, those in the far-flung areas cannot receive healthcare services and maximize their benefits. I hope that the government can truly see the disparity and gap in the accessibility of healthcare services across areas and develop plans and programs to ensure that those in disadvantaged locations have equal access to technology (e.g. internet, gadgets, and power) to those in the 'mainland'. Technology must be available to them so that they can read timely and accurate health information, helping them make informed decisions and promoting proactive (preventive) behaviors.

Teenage pregnancy presents a significant priority healthcare need due to its several impacts on both the mother and child. Many teen mothers avoid seeking necessary healthcare services throughout their pregnancy due to factors such as embarrassment, time constraints, lack of education, financial barriers, and insufficient awareness. This low health-seeking behavior significantly increases the risk of pregnancy-related complications and harm to the unborn child (Perez, 2020). According to Scheufler (2023), teen mothers face heightened pregnancy risks such as pre-eclampsia, anemia, and gestational hypertension. They also often grapple with challenges in maternal health, including issues related to body image, nutrition, and postpartum recovery. Emotional and mental health impacts are also profound, with increased risks for depression, anxiety, and stress exacerbated by social stigma and lack of support.

Given this, nursing informatics should prioritize the development of user-friendly digital resources to improve access and engagement. This involves creating health applications and online platforms that provide essential information about pregnancy, prenatal care, and postnatal support in diverse languages and formats. The advancement of telehealth can further facilitate access to care, especially for those who suffer from social stigmatization. Additionally, enhancing education through interactive tools within health information systems can raise awareness about the importance of regular prenatal care and address common barriers to care by offering clear, actionable guidance. Moreover, integrating support services such as mental health counseling and peer support networks into a single platform can offer comprehensive care that meets both the physical and emotional needs of teen mothers.

Among the healthcare needs of our community in Bulacan, the increasing number of stray dogs and cats necessitate the need for anti-rabies vaccination particularly in the age group of children. As such, the competencies of nursing informatics highly influence and somehow hinder the application of nursing informatics in the delivery of healthcare services. In a literature published by Darvish et al (2014), three competencies were identified including Computer Skills, Informatics Knowledge, and Informatics Skills.

Meanwhile, four levels of nursing practice were also identified including the beginning nurse and the experienced nurse. With the onset of the pandemic, it is a common knowledge and a shared experience that most nurses struggle to transition into the digital data management especially experienced nurses who are mostly advanced in terms of age. In relation to this, the identified Computer Skills competencies such as the documentation of patient care as well as the use of telecommunication devices presents as a struggle in the application of nursing informatics in the delivery of healthcare services.

In 2021-2022, an increase in the live births among women younger than 15 years old was observed (Bacelonia, 2024). I have also personally witnessed a number of adolescent pregnant women in my clinical duty last semester in PGH OPD, OBAS, and Ward 15.

Sadly, teenage pregnancy, the lack of sexual education, and contraceptive use are still rampant in this day and age. This is due to a variety of factors, ranging from governance to religious issues.

Despite that, some technologies have been studied to increase FP knowledge and access. One such study is by Dev et al. (2019), who utilized an Interactive Mobile Application for Contraceptive Choice (iMACC) to aid women in making more informed decisions. They found that most of the women found it helpful as it was both accessible and confidential. However, issues regarding digital literacy were also set forth.

With that said, we can see that accessibility, confidentiality, and literacy are important factors that influence the application of nursing informatics in this type of healthcare delivery, especially in our country, where accessibility and literacy are not at a favorable level. However, the confidentiality and importance of technologies like these are what would help bolster the implementation and usage of these healthcare delivery systems to a wider audience.

One of the most prominent healthcare needs in my community is sex education for adolescents. According to the Philippine News Agency, in 2022, there was a 35.13% rise in total live births among girls under 15 years old, from 2,320 in 2021 to 3,135 in 2022 (Bacelonia, 2024). Given that adolescents are known to be curious and tend to explore activities such as sexual intercourse, sex education is a must. Several factors influence the application of nursing informatics in the delivery of proper sex education to this group. Privacy and confidentiality must be the primary concerns of adolescents. They might fear that their parents and/or guardians have access to their sexual health information. Hence, informatic systems such as electronic health records must ensure confidential access to these topics, which are very sensitive in this age group. Moreover, sex education is still viewed as taboo in many communities. This may hinder adolescents from engaging in informatics tools for sex education. Telemedicine may also not be accessible to everyone since not all adolescents have gadgets or other tools at hand for online consultations. For some, it might not be comfortable to them as there is a tendency for other people to hear the conversation since the setting is not confined to the clinic or hospital. With the ever-changing healthcare system now influenced mainly by evolving technology, measures and actions should be taken in pursuit of the services to be accessible to everyone.

In my opinion, the healthcare need of people that should be prioritized on among young adults is mental health awareness. Various challenges and pressures faced by young adults, such as academic demands, career uncertainties, and relationship complexities, have a significant impact on their mental well-being.

One significant factor influencing the application of nursing informatics to mental health awareness among young adults is the stigma surrounding mental health problems. Despite ongoing public efforts to promote and advocate for this critical issue, there is still, undoubtedly, a negative connotation associated with mental health. While nursing informatics has provided accessible online resources for those seeking help, misinformation and negative perspectives on social media platforms remain prevalent. Consequently, the struggle to eliminate stigma, prejudice, and discrimination against individuals with mental illness has still a long way to go.

I suppose it is common to see diabetic patients in my community in the province. Nursing informatics can ideally be applied through electronic records, decision making systems, and patient engagement tools for improved care and outcomes. For example, keeping record of patients with diabetes in the community may prove helpful for decision making in care– in blood glucose levels, medication administration, and the like. It may also help the concerned healthcare practitioners cater and customize their treatment plans for the specific needs of these diabetic patients. Data management, privacy, and resource allocation may significantly influence the effectiveness and efficiency of these tools. Healthcare delivery systems may be enhanced and ultimately improve patients quality of life and health outcomes.

Interesting application to a population group I had not considered! Chronic non-communicable diseases such as diabetes take a lot of monitoring and interventions to help patients improve, so I think it is a good idea to use technology in such a way that the constant maintenance and treatment is made easier. This could likely also help in improving medication adherence and checking of changes in glucose, amongst other things.

Ali et al. (2020) identified several significant barriers that hinder the application of nursing informatics in delivering healthcare services to people with disabilities (PWDs). These include technological inaccessibility, limited skills and knowledge among healthcare workers, and poor service quality.

Accessibility remains a major challenge for PWDs, particularly those in marginalized regions and the Global South, where healthcare is often underfunded, resulting in compromised service quality and limited reach. Although the advent of telemedicine during the COVID-19 pandemic offered new opportunities, there is insufficient data to confirm whether these innovations have been inclusive or accessible for PWDs.

Moreover, ableism and societal biases against PWDs persist in healthcare despite existing laws like the Republic Act No. 7277 (Magna Carta for Disabled Persons) and Batas Pambansa Blg. 344 (Accessibility Law). These ingrained negative attitudes can translate into discriminatory practices among healthcare professionals, creating barriers to the effective use of nursing informatics and technology to enhance service delivery for PWDs.
Finally, due to ongoing discrimination and financial limitations, there is a lack of targeted training for healthcare workers in using technology to meet the specific needs of PWDs. This lack of education leads to the exclusion of PWDs from digital health innovations and results in poor service quality

This is especially true in using digital health systems, where selective populations are chosen, often forgetting to include PWDs. Hence, we should continue to advocate for inclusive innovations that cater to all populations.

In the Philippines, the effective management of noncommunicable diseases like hypertension and diabetes mellitus is significantly influenced by various factors related to nursing informatics. Firstly, access to and availability of technology play a crucial role, as reliable internet and digital health tools are essential for telehealth services and electronic medical records. In areas with limited technological infrastructure, these tools may be underutilized. Furthermore, training and education for both healthcare professionals and patients are critical; nurses must be literate in informatics systems, and patients need guidance on using health apps and monitoring devices. Data management is also pivotal, with the need for interoperability between systems to ensure seamless data integration and high-quality information. Cultural sensitivity and socioeconomic factors must be considered to make informatics tools relevant and accessible, overcoming barriers related to cost and digital literacy. Supportive health policies and adherence to regulations regarding data privacy and security further impact the successful implementation of informatics solutions. And lastly, patient engagement is enhanced by user-friendly tools, which help address technical challenges and promote effective disease management.

Several factors influence the application of nursing informatics in delivering effective healthcare services to this target group. One key factor is access to technology. For instance, they provide cellphone numbers for contact instead of offering online consultations, possibly because not everyone has reliable internet access, though more people likely have access to basic phones for texting. Another factor is data management, specifically how efficiently patient information is collected and organized. Additionally, regular evaluation of services and feedback, along with continuous updates to digital tools, are crucial for improving healthcare delivery.

I chose pregnant women as my target group. I have this neighbor of mine who was recently pregnant and shared her experience with our barangay health center. She mentioned that during her third trimester, she had difficulty keeping up with timely consultations for her checkups because she was often left at home without anyone accompanying her, and she tended to forget them, opting instead to stay at home to rest. This is where the factors influencing the application of nursing informatics come into play - the accessibility and engagement of pregnant women in accessing the care they need. In a study made by Woldemariam and Jimma (2023), they emphasized the importance of having Electronic Health Records (EHRs) in improving the quality of healthcare services delivery such as in data management. Applying this in the context of not only the example I have given in my community but the pregnant women in general, the adoption of this EHRs system could possibly enhance the accessibility of healthcare services and engagement of pregnant women with their prenatal care needs as they can access their health records themselves and can even be reminded of their check up or consultation schedules through automated messages or directly from their healthcare providers leading to better adherence to care.

Family planning is a critical healthcare priority in my community. The prevalence of teenage pregnancy and large families, coupled with the challenges of meeting daily needs, underscores the importance of addressing this issue. Despite the accessibility of technology, health education on family planning remains limited. While family planning methods are available in the local health center, community engagement in these initiatives is lacking.

Notably, the community members possess technological literacy, suggesting potential for leveraging technology to enhance health education efforts. While many of the factors needed for the application of nursing informatics are already available in my community, including internet access, healthcare infrastructure, and trained healthcare workers, the primary area that requires attention is community engagement.

Both in the community and hospital setting, I have noticed a great number of elderly people. I’d like to focus on this particular population group since they have increased vulnerability to chronic conditions and functional decline. Therefore, chronic disease management is a priority healthcare need for this specific population. This includes ongoing monitoring, individualized care, and support groups because this population is more likely to become forgetful and isolate themselves, and some may even become victims of neglect, which can make their conditions worse.

With this, nursing informatics can be applied through means of monitoring devices that track vital signs and medication adherence, enabling medical professionals to identify changes early and take appropriate action promptly. Furthermore, the easily accessible health information provided online by reliable sources may also aid in slowing down the deterioration in the health of the elderly. Lastly, EMR helps make referrals easier since data can be shared with another physician whenever the patient needs more specialized care.

However, despite these advancements in healthcare, several factors, such as access to technology and digital literacy, may have an impact on how nursing informatics is used to provide care to the elderly. Not all elderly people have access to technology, devices, online health resources, and applications. Furthermore, while some people may have access, not everyone possesses the necessary digital literacy to fully utilize the online tools that can enhance their health and lessen the likelihood of worsening their conditions.

My community, Isla Puting Bato has recently been affected by a tragic fire. Many people lost their homes and belongings. The current state of their evacuation center does not make things easier. Overcrowded and unattended, the residents vie for medical and social attention.

Social media enables me to publicize their plight in order to tap more people to donate relief goods and aid. However helpful donations are, there is still the matter of acquiring, delivering, and distributing them. As it is right now, it is quite difficult to influence people to donate, and that in itself is a problem applicable under informatics.

The nature of the people's problem (accidental fire) is nothing new to the healthcare industry. It's commoness is an asset in itself in the sense that it may be easier to find existing nursing informatics systems applicable or even inspired by it. Ideally these should include systems where their situation reaches a lot of people, and the amount of donations that are procured for them is maximized.
