Most of my learning outcomes for my unit of instruction require a solid foundation in the cognitive domain, so tests with MCQs are still useful. The in-service exam also tells us how individual residents fare against other residents in the country, so it's a good resource for benchmarking and continuous improvement.

I agree about the utility of MCQs, but looking back, In-service examinations, at least for surgery may be wanting with regards to item construction and even alignment with outcomes.
