Course Description and Objectives
ORTHO 260 – Internship in Orthopedics

Course Description: This 2-week rotation will offer the UPCM LU7 (Track A) and Post-graduate interns a hands-on experience in managing emergency and elective Orthopedic cases seen in the clinics, wards and operating room. This will also provide resources to review basic knowledge as well as the advancements in managing the whole spectrum of musculoskeletal conditions.

Knowledge
- accurately diagnose common acute and chronic MSK conditions based on concise evaluation
- integrate the natural history of conditions in prognosticating and providing advise on proper management of Orthopedic diseases
- understand the basic steps of an orthopedic surgical procedure
Skills
- perform concise physical examination to arrive at a diagnosis
- apply proper basic splints and casts in the management of extremity injuries
- assist orthopedic surgical procedures
- perform appropriate care for a variety of wounds in the spine and extremities
Attitude
- demonstrate empathy and compassion to patients with MSK conditions
- exhibit professionalism and team-work among colleagues
Orientation
There will be an orientation session for every block before or on on the first day of the rotation.
The specific time of orientation will be set by the block's liaison officer and the department's resident monitor.
The students may review the contents of the orientation presentation anytime and consult with the resident monitor or course coordinator if there are concerns.

Splinting and Casting Session
At the end of this activity, the students are expected to:
- Integrate the principles of immobilization with the indications for applying different types of limb splints and casts
- Watch the splinting videos prepared by Ortho PGH.
- Demonstrate the application of a long arm cast using plaster of Paris

Surgery videos galllery
This section curates videos of common surgical procedures that interns assist during the rotation.
After going through the videos, the student is expected to:
- Be familiar with the steps in performing common Orthopedic procedures
- Describe relevant indications in doing Orthopedic surgery
Learning from this section will be evaluated during surgical assists and through questions included in the end-of-rotation exam.

- Indications for surgery
- Steps in evaluating proper reduction and implant placement when the fracture is not visualized
- Post-operative plans
Points to focus on:
- Evaluation of ankle injury
- Indication to use an external fixator
- Planning and marking prior to cutting
- Safe placement of percutaneous pins
Points to focus on
- Options for managing a peritrochanteric fracture
- Approach to fractures around the hip
- Positioning of a hip prosthesis close to anatomic

- Principles in performing fasciotomy
- Review of upper extremity anatomy
- Techniques in safe dissection around the forearm, wrist and hand
- Principles in managing deep space abscesses

- Indications for procedure
- Maneuver to perform reduction
- Splint options after reduction
- Predictors of DRF instability
Points to focus on:
- Indications to do surgery
- Careful soft-tissue dissection
- Indirect reduction (aided by implant)
Community Orthopedics: Real World Stories

Small Group Discussions
There will be 2 SGDs scheduled within the rotation, scheduled on the Friday afternoon of each week.
The default set-up is face-to-face with the flexibility to shift to hybrid or pure online.
At the end of these activities, the student is expected to integrate the core knowledge in Orthopedics and apply the principles in case-based discussions of actual and hypothetical patients.
