I do not recall a personal experience with witnessing fabrication or falsification of data among my colleagues or data collectors. I think it is difficult to detect this especially among data collectors doing field work in study sites that are remote from me. I have to rely on their honesty and the astuteness of the field coordinator who is supposed to check the data collection forms for accuracy.  I did have an experience of consent forms with missing signatures of participants or witnesses and data collection forms with missing data. I returned all these to the field coordinator and insisted on wet signatures on all consent forms and completion of missing data. They complied with completely signed forms and data  but I have no way of knowing if all the signatures and data were legit. If the signatures were not from the actual participants/guardians and the data not accurate, then this would be a form of fabrication.
