The issue I have encountered is where co-managing services cannot decide on who will write-up a case report. Our department has come up with guidelines regarding this matter and I concur that talking amongst the authors can help determine the order of authorship.
Perhaps you can share this guideline? No personal experience but I have heard of some instances.
