Going through the health systems goals and challenges with their respective outcomes rooted from the eHealth vision, one can argue that all of the propositions are ideal in achieving desired health outcomes for eHealth. The improvisations that each component runs down are the best targets that truly represents an effective and accessible eHealth program. But as perfect as these projected outcomes are, they’re only as good as they sound only in the absence of circumstances that can impede the actual accomplishment and mobilization of such objectives. In a meta-analysis conducted by Tossaint-Schoenmakers et al. (2021), three factors were synthesized and deemed important for the success of eHealth integration into the organizational structure and daily care processes— role of the care benefactor, technology, and deployment of human resources. Each of these factors play a very pivotal role towards the success of eHealth, such that noncompliance could negatively affect the entire structure of eHealth from its organization, processes, up until its outcomes. The challenge of commitment with the involved healthcare personnel also poses a threat in ensuring an effective and efficient development of eHealth systems. Likewise, the lack of interoperability among various health information systems create a widening gap not only among the supposed beneficiaries but also within those that supposedly manage them for better utilization and application (Qureshi et al., n.d.).

I completely agree with you, kuya Drix! Achieving the outcomes for health are affected by the factors such as technology and role of the care benefactor as you have stated. Ensuring commitment not only from the the healthcare personnel, but also from the leaders that formulated this innovation and guaranteeing that there would be interoperability between health information systems are crucial in achieving an effective and efficient health care delivery. I definitely agree that the lack of interoperability among various health information systems hinders the full utilization of eHealth. This reminds me of a community meeting I attended where a health center used online appointments, but some barangays under its jurisdiction did not comply due to citizen literacy and resource limitations. Consequently, tracking patients who visit health centers becomes a double task, as healthcare workers must review both online appointments and physical slips.

I agree with your answer Drix, for eHealth to function properly, interaction between the processes and variables within the system need to be seamless and efficient for the goals of eHealth to be met. This becomes a challenge because if the process or whatever variables in the system do not properly interact with each other, it may cause the system to be inefficient and maybe ineffective in delivering patient care.

EHealth outcomes are achieved through collaborative work between several health systems. Its emerging accessibility to different geographical areas also increases the demand for healthcare services. A major challenge that I can perceive is people's digital illiteracy. Many have access to various health information, but only a few can understand and apply it to their lives. In fact, according to an article by Abarca (2023), the Philippines has the highest number of digitally illiterate people. The spread of misinformation and disinformation is apparent, which leads to people seeking inappropriate health behaviors, such as self-medicating. When it comes to eHealth services, they may or may not be able to effectively utilize the tools provided for them, as they tend to seek the “easier” way of giving solutions to their health problems.

 

Thus, this disregards the vision of eHealth when it comes to more equitable and responsive healthcare. If we only improve the access to eHealth for those in remote and disadvantaged areas, we are not sure if they can understand what we are providing for them. If we are only after widespread access to eHealth services and we are not doing something after the digital illiteracy of the community, we are not providing them quality care that is the same as the physical presence of healthcare providers.

For instance, several clients in the community do not know how to navigate and access the MHD website, which affects their health-seeking behavior and appointment plans. The eHealth outcome is a noble pursuit that advocates for widespread access to health through technology; be that as it may, several challenges such as digital literacy and technological concerns still impede this goal's fulfillment.


In an article by Evio & Bonito (2024), the barriers encountered were multi-dimensional. Starting from individual characteristics, digital competence and resistance to change were the common barriers as many of the health workers that use eHealth in their respective institutions found it difficult to adapt to the technology because of their lower digital literacy than their younger counterparts. Content-related factors such as eHealth design, complexity, and reliability also caused difficulties (Evio & Bonito, 2024).

Similarly, in Marcelo et al.'s (2022) study which focused on the factors influencing the usage of RxBox, common themes of criticism on the telemedicine device experience include age and device issues. These criticisms stemmed from the users' lack of readiness to accept the new technology attributed to their age and lack of digital literacy background. Negative reports about the service was also a concern as the device had instances of technical issues (Marcelo et al., 2022).


With that said, it is clear that the barriers to the eHealth outcome are multifaceted; however, the most apparent challenges based on available literature point to disinclination to change and technological and design issues. Thus, this requires multisectoral effort starting from good governance to appropriate training.

From the study you have shared by Evio & Bonito (2024), the ease of use of eHealth designs was identified as a barrier to the use of eHealth service. Perhaps a sociological explanation for this is that individuals tend to revert to comfortable and trained practices when new experiences are stimulated. With the integration of eHealth in the Philippines’ healthcare delivery system, resource management poses a major challenge toward achieving its outcome. This challenge becomes even more critical as the country’s healthcare system faces financial limitations and the uneven distribution of resources across various institutions.

First, eHealth’s complex technology comes as a major challenge for healthcare facilities to maintain relatively expensive ICT equipment like desktops and laptops (Britiller et al., 2023). These devices often require regular updates, repairs, and replacements, further straining the limited budget of implementers.

Aside from this, the feasibility for the eHealth system to achieve the eHealth outcomes also requires the need for a stable ICT infrastructure, particularly having a stable Internet connection and available power supply to support it (Evio & Bonito, 2024). Without these, the system’s reliability on its exchange for health information between the healthcare team across institutions may be compromised.

Additionally, resource availability to support the workforce also comes into the integration of the eHealth system (Evio & Bonito, 2024). The successful implementation of this system depends both on the infrastructure and the upskilling of staff through seminars and training, ensuring that even the older healthcare staff can navigate themselves through the eHealth technology.

The implementers’ capability to support these resources would be a major challenge for the capability of the implementers to seek solutions and integrate the eHealth system in the Philippines for better health outcomes.

Looking into resource management, there are still indeed a lot of improvements to make, particularly with stable internet connection and available power supply which are both challenges in the country at the moment. I appreciate that you mentioned that older healthcare staff should also be able to navigate through the eHealth technology. As eHealth continues to evolve, we should ensure that no one is left behind; it should not be exclusive to the "techy" generation just because we think that it's only them who can navigate through these changes. The achievement of eHealth outcomes is influenced by different factors that encompass the implementor, patient, and healthcare workers who participate in the process.

The implementation of eHealth programs is a significant consideration or factor in the success of eHealth outcomes. In the study by Granja et al. (2018), it was discovered that patient-centered care, management, and improved diagnosis have positive impacts on the quality of care and satisfaction of the patients. Hence, improving eHealth outcomes.

However, health literacy is a major barrier to achieving eHealth outcomes. Health literacy describes the capability of a person or individual to seek, understand, and utilize services and information to create health decisions for themselves and others (CDC, 2023). An individual who is adequately health literate could manage their disease well because they can participate in their health decision and communicate with the healthcare team concerning disease management (Hasannejadasl et al., 2022a).

With the advent of digital tools and the emergence of eHealth, people with low digital literacy skills are negatively affected because they cannot fully understand what is presented therein; hence, misinterpretation and misunderstanding may occur which compromise the patient’s welfare.

This suggests that digital health literacy is a key factor in the achievement of eHealth outcomes as the goals of improving the patient’s conditions would not be achieved if the patients are not fully equipped with the appropriate skills in discerning the information relayed in eHealth platforms (Hasannejadasl et al., 2022b).

Currently, different health centers use various systems and data formats, hindering the seamless information exchange between facilities. This lack of interoperability complicates care coordination and decision-making. In addition, while eHealth aims to extend healthcare services to remote and underserved areas, this goal relies heavily on basic infrastructure, such as electricity and internet connectivity. Unfortunately, many rural regions in the Philippines still struggle with unreliable or non-existent access to these essentials. Lastly, proficient healthcare workers using digital tools and systems are needed. Poor governance, lack of proper policy implementation, and poor compliance in healthcare sectors outline the Strategic Framework. It encourages governance, legislation, and compliance but shows broken systems and slow policy adaptation signifying poor performance. Integration of ICT into the healthcare sector for equal care delivery is the basic objective; thus, it calls for strong governance for uniformity between urban and rural settings (DOH & DOST, 2013).

Limited interoperability standards limit the secure interchanging of patient data among health information systems. Added to this are insufficient infrastructural developments that exist in most rural areas. This diminishes the productivity of eHealth solutions (Marcelo et al., 2020). The poor training of healthcare professionals on the usage of eHealth technologies, including telemedicine and EMR systems, also limits their widespread application. For example, the Community Health Information Tracking System, in spite of achievements in some areas, it also had inconsistent training and resource support (Ongkeko, 2017).

The plan will break these barriers with more stringent regulations and authority, better training and constant assessment that is capable of institutionalizing effectively eHealth solutions across the country (DOH & DOST, 2013).

As the world shifts from traditional pen and paper, the Philippines plans to integrate technology in providing right health information and managing a health system that will be more effective and efficient. However, considering that the Philippines is a third world country, there are challenges that need to be addressed in order to achieve all desired outcomes.

One of the identified barriers focuses on the digital competence of the Philippines. Since there is no guiding blueprint or any government body that oversees digitized health systems, these systems have scattered into different areas, which only caused fragmentation of information and straining of frontline workers (Marcel & Marcelo, 2024). Furthermore, Sylim et al. (2022) identified that there are issues related to interoperability of eHealth in the Philippines and one of which is the lack of an institutional mechanism to regulate electronic medical records

Another identified issue focuses on the capability of the people who will operate these electronic systems. These include an individual’s experience or training with information and communication technologies and their readiness for change (Evio & Bonito, 2024). The theory of Diffusion of Innovations by Rogers stated that not all people adapt an innovation at the same pace; there are factors such as complexity of the new innovation that influenced their adaptability.

These barriers can be alleviated by the support from the country’s leaders as they have the power to shape the healthcare system and most importantly they can help people adapt to this innovation.

The creation of ehealth aims to address accessibility of health care services among Filipinos across the country. In achieving this, there is an integration in the governance, the healthcare team, and the general population. Although I believe that the initiative is timely especially in today’s time where more people are becoming technologically literate, the main challenge lies in the cost that comes along with it. Creating and maintaining ehealth requires investing, and given the Philippines' current economic situation, it is difficult to sustain the initiative. Moreover, the majority of the Filipinos residing in the country are part of the lower-income class, hence, it is impractical for them to purchase a device, install wifi, and pay for telehealth consultations in order to maintain their health. Ultimately, ehealth is a progressive initiative that has been very helpful especially in the pandemic, but financial problems especially in the underserved community continues to prevail.

Our countrymen's current financial situation can be a hindrance to accessing eHealth, and this is why I believe the government has to be more proactive in creating strategies that can make eHealth more accessible, even to the underserved communities in the country. One thing that I can think of is the provision of devices, topped up with cellular data every month, to the barangay health workers in the country that will allow people to connect to them and ask for assistance to access eHealth services. Despite the growing technology in the Philippines, particularly in healthcare, there are still numerous obstacles that prevent citizens from fully accessing eHealth services.
According to the Philippine News Agency (2024), the country’s geography significantly impacts the challenges in achieving positive eHealth outcomes. It states that nearly 60% of households still lack internet access. This data shows that more than half of Filipinos do not have access to the internet. Strong digital connectivity will enable them to effectively explore various programs, electronic records, and services that are readily available on the platforms. There are still numerous challenges that our community continues to face up until this moment such as the digital literacy of the people. So there are a lot of factors that needs to be taken into consideration but I really do believe that we, as an ISKOnars, would be able to contribute to this advancement. By doing so, we can create a community where citizens have access to eHealth and are well-informed about how to use it effectively.

Majority of the people living in the Philippines are of minimum wage workers hence I would agree that some, if not all, have difficulty in adapting. Furthermore, although lack of internet access can also be a challenge, I would also like to add that if there are people who have internet access, geographically speaking, the philippines is composed of an estimate 7,000 islands making it difficult to build infrastructure like cables and wires that help speed up the connection.

I think one of the major challenges to achieving eHealth outcomes is the need for more funding to ensure the continuity of implementation and the achievement of eHealth goals. One of the core aspects of eHealth is ensuring that health is accessible for all through partnerships, eLearning, and cost-effective strategies.

For example, telemedicine has become more prevalent, especially at the peak of the pandemic. This allowed people to meet with their healthcare providers to address their healthcare needs from the comfort of their homes. However, the availability of this service, and other strategies like eLearning, is limited to those with electronic devices, internet access, and the like, which contradicts the purpose of eHealth.

To address this issue, the Philippines could undertake a large-scale infrastructure project to improve national broadband and healthcare facilities. Furthermore, increasing the doctor-patient ratio should be done while considering the doctors’ needs to make healthcare more accessible to the people. Lastly, the government must be transparent with budget allocation and ensure the protection of digital information (Dela Cruz & Tolentino, 2021). Considering the huge role of the government in this topic, the people should hold them accountable for any discrepancies observed.

The eHealth outcomes are envisioned to benefit health consumers, healthcare providers, healthcare managers, policymakers, and researchers. In pursuit of providing safer and quality care, better decision-making, and effective programs to implement, multiple health system goals or challenges are set to conclude with the desired eHealth outcomes. However, challenges and barriers are expected in achieving these goals.

A study by Evio and Bonito (2024) highlighted that there are three interrelated factors forming barriers to eHealth adoption: contextual, process-related, and content-related problems. The major contextual factors largely refer to individual attributes, such as attitudes toward technological change and degrees of digital competence found among healthcare workers and patients. As for its process, eHealth remains highly dependent on stable technology that creates stringent requirements in terms of internet connectivity and stable power supply when it comes to attaining health outcomes. Process-related issues also present challenges with the complexity of eHealth systems that do not easily integrate with the existing health systems. Poorly designed, non-user-friendly, or rigid eHealth platforms can be problematic for both service providers and users.

This is premised by Marcelo and Marcelo (2016) regarding the problem of data interoperability. The fact that there are no standardized health information systems in different healthcare institutions of the country promotes the barrier to efficient data sharing, which is crucial for coordinated and comprehensive patient care. Overall, without data integration, eHealth systems cannot function efficiently; hence, they might not be able to have an effect on healthcare delivery to their full potential.

These multifaceted challenges addressed in infrastructure, system design, and data interoperability allow the Philippines to take steps towards its eHealth aspirations.

However, one of the elderly residents I was able to interview in our rapid appraisal expressed that she does not know how to use gadgets and often has to ask for help from her daughter. Such situations highlight the challenges in the achievement of eHealth outcomes and, as you have mentioned, the need for literacy programs to ensure that everyone can maximize the utilization of these health resources.

The Philippine eHealth strategy framework has proved beneficial in facilitating widespread access to healthcare services and information. However, several barriers and problems exist in delivering eHealth outcomes. One of the highlighted main challenges is the interoperability issue. The ability to smoothly interact and share information becomes a problem and a crucial hurdle in achieving eHealth objectives (Salisi et al., 2016).

There is also a need for more resources and sufficient finance, and long-term technological integration has yet to be achieved. Implementing interoperability solutions can be costly, requiring extensive skills and resources. Most still lack access to proper internet connections and digital gadgets, hindering eHealth outcomes. There is also no connectivity with other systems, and is incompatible with others, particularly in different locations (Salisi et al., 2016). Furthermore, several healthcare institutions have distinct business procedures and decision-making strategies, which increases complexity and leads to unsuccessful implementations. There are organizational silos in which systems have grown into diverse areas, putting a strain on healthcare personnel to encode data for public health benefits (Marcelo & Marcelo, 2016). As a result, when providing patient-centred coordinated care via eHealth, these challenges should be addressed through an overall governance structure and guiding framework. To improve the efficiency, efficacy, and accessibility of eHealth programs, the country must consider facilitating interoperability.

Definitely, eHealth is a novel concept that can push the barriers of how we understand healthcare and how we can use it to improve healthcare for our patients our patients. Tossaint-Schoenmakers et al. (2021) wrote an article describing the challenges we may face in its use. They found three key principles: the care that the patient needs, the capabilities of eHealth technology, and the available human resources. When one of these factors are not at available at full capacity, eHealth outcomes cannot be achieved. The care of the patient needs to be possible and available to them so that they may be able to receive it. The technology to help them access this should be organized in such a way that the care process is left undisturbed. The healthcare workforce must have the capacity to accommodate not just their regular clients, but also those who need their help via eHealth. With all of these in mind, eHealth must be developed in such a way that takes into considerations the limits of our technology and our people.

(2020), healthcare providers, particularly those in primary care settings, sometimes face obstacles when using telemedicine due to a lack of training, inadequate internet access, and a scarcity of necessary equipment. These infrastructure constraints impede the efficient use of eHealth tools.

Furthermore, the initial hostility to the Community Health Information and Tracking System (CHITS) demonstrated the significance of bridging the digital divide among healthcare practitioners. Ongkeko (2017) observed that many healthcare providers' lack of computer literacy restricted the use of electronic medical records. Furthermore, economic constraints in healthcare facilities have limited the capability for regular software updates and maintenance, jeopardizing the long-term sustainability of eHealth programs.

Finally, Go (2020) highlighted systemic barriers, such as regulatory limitations, technological disparities, and resistance from healthcare professionals accustomed to traditional practices, which continue to impede widespread utilization of telehealth services in the Philippines, particularly in underserved communities.

The successful implementation of eHealth results in the Philippines requires a coordinated effort to solve the various hurdles posed by training shortages, technological limitations, regulatory barriers, and reluctance to change. Only by eliminating these barriers will the full promise of information and communication technology (ICT) in healthcare be realized, improving access, quality, and efficiency of care.

The eHealth system here in the Philippines is constantly developing to improve the efficiency and quality of healthcare. However a major challenge to achieving the eHealth outcomes are issues with internet access and data privacy. While we recognize that eHealth has numerous benefits, we cannot deny that these challenges are magnified by the lack of implementing structured and comprehensive policies.

Recently, there are guidelines released by the DOH and PhilHealth to further facilitate the implementation of eHealth systems in our LGU’s. However, Dr. Meredith del Pilar pointed out that the policy on the national level has not been felt at the community level. This just shows how the policies being implemented are not effective. She emphasized the urgent need to upgrade digital network infrastructure to support the initiatives of eHealth systems (University of the Philippines Manila, 2023).

That is why we need to demand our government to fund the healthcare systems, and put effective and structured policies to ensure that the people can benefit from the eHealth advancements, ultimately leading to healthcare for all.

With a vision of a more prosperous eHealth initiative in the year 2020, our organizations and individuals have been steadfast in achieving the goals and initiatives stated in the framework. Nonetheless, with the current setup of healthcare in the Philippines coupled with social determinants of health that seem to be continuously deteriorating in several communities in the country, it is safe to say that we are far from thoroughly attaining our ideals.

In the recent fieldworks we have conducted in our chosen communities for the course N119, alongside the previous experiences garnered from our N12 course, what I have sought as a major challenge in the achievement of eHealth outcomes is the struggle to use and/or acquire technological devices for the elderly population. With the Manila Health Department’s shift to the online appointment system, the elderly individuals have been mostly affected by the eventual changes brought about by the difficulties in utilizing the new technology. It has been expected that the younger generation is more adept in using these ICT tools, considering that they have been born and raised in the digital age (Bujnowska-Fedak & Pirogowicz, 2013). Those elderly individuals living and/or interacting with the younger generation, such as their own children and/or grandchildren, may have an advantage as compared to those who live alone. Due to difficulties in booking appointments and some specific health centers no longer allowing walk-ins without digital confirmations, members of the older generation are deprived of the health services, ranging from medications, check-ups, and laboratories. While we continuously align our visions to the changing times of innovation, it is more important that we do not leave any population groups behind, especially those who are considered vulnerable and susceptible.

One major challenge to achieving the eHealth outcome is the lack of internet connectivity in some areas of the country (Montemayor, 2024), compounded by the lack of government initiatives to solve this. For instance, in Iloilo, there are households where you have to walk for at least half an hour from the main road to reach them. These areas would usually rely on the signal obtained from the main roads, which hinders them from readily accessing information. There is even a story featured on the news during the height of online classes, where a student had to trek up and down a mountain just to attend online classes, further highlighting the need to improve internet access in the country.

Moreover, in these areas, not everyone is adept at the use of electronic devices to access the internet, which makes accessing eHealth even more difficult. This issue was also identified in the study of Lu and Marcelo (2021), where they pointed out that uncoordinated investment in ICTs in health is a key challenge to eHealth in Southeast Asia. This only shows that eHealth will not be successful if the government does not initiate efforts to make internet accessible to everyone in the country. Initiatives to educate people about the use of the internet are also crucial because if they do not understand how eHealth works, they will not see its potential to help them better access healthcare services.

Although barangay health workers can help alleviate this problem, some BHWs cater to multiple barangays, making it impossible for them to reach every single household in a short period of time just to connect them to eHealth and teach them how to utilize it. Recently, plans like the utilization of unused frequencies between broadcast TV channels to provide affordable internet access is being explored by DOST. However, without sufficient government support, this will still remain a potential solution to the problem. Achieving the eHealth outcome requires consistent and effective collaboration between the government and various sectors to ensure that eHealth services will be provided even in far-flung areas of the country.

This is such a great point, Vi! While the nationwide use of eHealth technologies is certainly an attractive idea, the reality is that there are many people who do not have access to internet services and may not be able to utilize eHealth technologies. Even in some of the communities we visited during our community rotations, many people complained about difficulties in online registration for health center visits, which would oftentimes serve as a barrier to their health-seeking behavior. This is definitely a factor we have to take into account when considering how to make eHealth more affordable and accessible in various areas of the country.

Nearly half of the population worldwide is unable to access healthcare services, as per UN report (Labrique et al., 2018, as cited in Nah Asah & Johan Kaasbol, 2023). Over 800 million people are only allotting 10% of their budget for medical expenses. This, in turn, hinders accessibility to healthcare (WHO, 2005, as cited in Nah Asah & Johan Kaasbol, 2023). In this regard, the World Health Organization encouraged world leaders to incorporate eHealth in the delivery of services.

The implementation of eHealth holds a promise of change. However, there are certain barriers that block the achievement of its expected outcomes. One of these is the limited number of resources to support infrastructure, internet connectivity, and digital literacy (Archer et al., 2021).

eHealth technologies have been considered as “key enablers of UHC”, but its effectiveness and efficiency are minimized as a result of the barriers (Nah Asah & Johan Kaasbol, 2023). In this sense, the need for infrastructures arises because it significantly ensures good internet connectivity, which are essential for telemedicine and accurate documentation of data (Klug, 2020, as cited in Nah Asah & Johan Kaasbol, 2023). In the same way, a strong foundation of digital literacy on eHealth will also “patients’ health-related knowledge and behavior, facilitate information exchange between healthcare providers and patients, and improve coordination and continuity of care while reducing the cost of healthcare delivery” (Nah Asah & Johan Kaasbol, 2023).

Apart from the high cost of electronic devices, the day-to-day expenses brought about by the usage of an internet connection introduce financial limitations, especially in developing countries like the Philippines.

Meanwhile, Evio and Bonito (2024) highlight multiple factors including stakeholder engagement among the factors that influence the utilization of eHealth in the country. In the same study, other challenges were identified such as the proficiency of healthcare professionals in using electronic devices. According to the study, those who are familiar with the usage of these devices such as younger staff are more likely to utilize eHealth while the older staff tend to raise concern, particularly on the difficulty experienced in the adjustment. To support the claim of Rosales (2019), financial and technical support was also identified by Evio and Bonito (2024) as part of a “third-party involvement”. While lack of resources has been a prevalent issue in the Philippine healthcare setting, the implementation of eHealth will most likely rely on the financial support given by their respective Local Government Units.

If eHealth systems are not well-tailored to fit the specific workflows and organizational structures of healthcare settings, they risk overburdening healthcare professionals rather than easing their workload. For eHealth solutions to be effective, care processes must be adjusted, human resources must be efficiently managed, and continuous coordination and communication are essential throughout the implementation process. Without these critical adaptations, the desired benefits of eHealth may not be fully realized.

The national eHealth vision was “by 2020, eHealth will enable widespread access to health care services, health information, and securely share and exchange client’s information in support to a safer, quality health care, more equitable and responsive health system for all the Filipino people by transforming the way information is used to plan, manage, deliver and monitor health services.” However, it is evident that up to this date, there are still significant issues in the implementation of eHealth in the country.

One of the major barriers in eHealth implementation is that many Filipinos lack the technical skills to adjust and adapt to eHealth environment. In addition, not everyone has access to devices and good internet connection. While eHealth aims to be a more efficient type of health care delivery for all Filipinos, it would not be as efficient if only those with ICT skills, the young, and those with good internet connection and devices can utilize it. There would be concerns when it comes to accessibility. Its design features should also be user-friendly. Another problem lies sustainability and funding. For eHealth to provide accurate, reliably, and timely data to the Filipinos, it needs to be continuously updated. It requires financial resources for its continuous operation and development. It is also crucial that the government prioritize eHealth. While there is a clear vision for it, they also have to address the barriers and ensure that all Filipinos can utilize it.

Many health system goals or challenges alongside their desired eHealth Outcomes were delineated by the Philippine eHealth Strategic Framework under the categories of Health Consumers, Health Care Providers, and Health Care Managers (Department of Health & Department of Science and Technology, 2013). Many challenges hinder the achievement of these eHealth outcomes and one of the major challenges especially for health consumers is their inaccessibility of health services mainly it is because of their financial status or even due to belonging to remote areas where the nearest hospitals are far.

In a third-world country such as the Philippines, it is a known fact that not all population groups (especially those who are deemed "vulnerable" and from geographically isolated and disadvantaged areas [GIDA]) have easy access to healthcare services, such as primary care. According to a PIDS study, about half of Filipinos do not have access to primary care facilities, and in the year 2020, only 50% of Filipinos could access primary care in the standard time frame set by the government. In addition, public health spending is at its lowest in the Philippines in recent years. In that regard, it is also a widely known fact that with the implementation of eHealth outcomes come challenges that may pose great difficulty. This includes a lack of access to technology and infrastructures/innovations where eHealth outcomes are heavily based. Many urban-poor and rural areas do not have the internet connectivity and technologies that are needed for the effective implementation of telehealth or e-medicine services. This demarcation may make health disparities even more evident, especially for vulnerable and disadvantaged population groups, if not addressed. In addition to this, other challenges include a probable lack of digital literacy among people and a lack of public health funding. Again, if all challenges are not addressed in a timely manner in consideration of the implementation of eHealth outcomes, health disparities may become more evident and pronounced among Filipino people.

The eHealth outcome highlights the use of technology to improve healthcare access among the people. However, problems related to digital illiteracy and adequate funding are major challenges, especially for a developing country like the Philippines. Digital illiteracy becomes a barrier in the complete acceptance or full implementation of eHealth — not just for patients but also for healthcare providers. Bali (2019) highlights how this poor awareness regarding the proper use of ICT in healthcare and the benefits it offers can create resistance among people to adopting eHealth, thus resulting in a paradigm shift. With this, economic investments are crucial in addressing these key challenges. Besides, having sufficient financial resources and funding is one of the backbones of addressing digital illiteracy. This aids in the need to invest in training programs for healthcare providers regarding the proper use of ICT in healthcare to fully maximize its benefits. At the same time, investment must also be made to provide technological infrastructure, particularly those in rural areas, to ensure access to eHealth is comprehensive and inclusive. Therefore, even with a clear objective to adopt eHealth, there wouldn't be any progress, and the vision would only be a vision if there were insufficient funds to address issues like lack of infrastructure and digital illiteracy.

For brief recall, the two healthy system goals/challenges from the Strategic Framework are 1) Health Consumers: Safer and quality health care, 2) Health Care Providers : Make more informed decisions, and 3) Health Care Managers, Policy Makers and Researchers : Effective program/research development, implementation and monitoring. In summary, the goals were has specific desired outcomes for the health consumers, providers, and the governing bodies of healthcare systems.
Referring to the study of Marcelo et al. (2020), a reference from the study guide, the peer and resource supports on training and technology after the implementation of the Telemedicine significantly determines the adaptation of the eHealth service. In support of this study, Leochico et al. (2020) have consolidated challenges in human, organizational, and technical aspects of implementing the eHealth service in the Philippines. The most common are lack of acceptance, of telehealth services, lack of national e-health policies or laws, and lack of internet access. Incorporating the two studies mentioned, the problem on resource support on training and technology is apparent in the healthcare system of the Philippines. There is lack of priority of both the government and health institutions in directing forward advancements on eHealth services such as lack of governmental policies. Moreover, the limitations on economic and geographical coverage of internet access pervade in every part of the Philippines. In today’s age internet is crucial in the bridge between eHealth services (ex. health support websites) and the intended clients.

The limited availability of high-speed internet in low middle income countries significantly hinders the widespread adoption and use of telemedicine services. Thus, a strong telecommunications infrastructure is essential for the effective and reliable delivery of telemedicine services. Despite the Philippines attempt to adapt an electronic/digital system for healthcare, a major challenge to the achievement of this outcome is lack of technical skills of healthcare providers and users. This limits the ability of both the providers and patients to utilize the implemented platforms.

Until now, not all healthcare providers are fully equipped with the knowledge and skills needed to manipulate and operate the system.Their readiness for change is also often a problem as they lack the confidence to adopt the new technology implemented. They report feeling hesitant to use the new technology as they are unfamiliar with the operations. Thus, this leads to the underutilization of the technologies and the weakening of the eHealth system. In turn, patients who are also digitally illiterate may refuse to utilize and access the system, leading to their inability to engage with the platforms and misunderstandings of health information.

In summary, the challenge boils down to the individual’s characteristics, specifically their readiness and digital literacy, which can be solved through proper training and creating user-friendly interfaces.

Personally, I think one of the biggest challenges in achieving eHealth outcomes is the limited access to digital infrastructure and resources, especially in underserved areas. In places like Metro Manila, it may be possible to implement these systems effectively, but in remote areas, access to digital media remains a significant obstacle. A clear example is internet connectivity. While the internet in the metro is reliable and fast, in more distant areas it tends to be unstable, which limits access to modern devices and prevents many communities from fully benefiting from eHealth services

The Donabedian Structure-Process-Outcome (SPO) framework highlights the need for strong organizational structures, effective processes, and measurable outcomes for eHealth to succeed. However, many countries face significant obstacles in these areas.

Structurally, the lack of reliable internet access, advanced medical equipment, and skilled personnel often hinders eHealth integration, especially in low- and middle-income nations. Even in well-resourced countries, resistance to adopting new technologies and digital literacy issues can impede progress. Process-wise, health care workers need proper training to integrate eHealth into their daily workflows, but many systems, already under strain, lack the capacity to provide this support.

The Philippines is a prime example of how these challenges manifest. In rural areas, poor internet connectivity and outdated infrastructure prevent the effective deployment of telemedicine and electronic health records. Overworked and undertrained health workers, especially in public hospitals, struggle to adapt to these digital solutions. Additionally, low health care spending limits the ability to realize the long-term benefits of eHealth, such as improved efficiency and access.

Despite the integration of policies and research on eHealth services, there remains a significant gap in the achievement of desired outcomes. I think a major challenge to the achievement of such eHealth outcomes in our country is our weak health system. According to Mengiste et al. (2023), low and lower-middle-income countries (LLMICs) often experience shortages of healthcare providers, lack of education opportunities, and governance issues that limit access to services, particularly in rural areas. These barriers continue to hinder the effective delivery of healthcare services and prevent the successful implementation of eHealth technologies in LLMICs, such as the Philippines.

In addition, socioeconomic barriers further complicate the situation, as not everyone in the Philippines has access to the internet. For many, online health services may not be affordable, and the lack of proper digital education leaves large portions of the population unable to benefit from eHealth innovations. These issues disproportionately affect marginalized communities, particularly those in rural and underserved areas, where internet access is limited and healthcare services are already scarce.

A major challenge to the achievement of eHealth outcomes is the lack of digital literacy. While digital literacy can be acquired through education, the existing generational and digital divides within society pose significant problems.


According to Campanozzi et al. (2023), the effectiveness of telemedicine outcomes is highly dependent on digital literacy. This is because digital literacy encompasses a range of skills, including basic computer skills, navigating health information, online safety and responsible use of social media. 


A deficiency in any of these areas can significantly hinder an individual's ability to fully utilize eHealth services.For instance, someone lacking basic computer skills may struggle to schedule appointments, access electronic health records, or participate in virtual consultations. Similarly, individuals with limited health literacy may be overwhelmed by online health information, leading to confusion, misinterpretation, and potentially harmful health decisions. Addressing the digital literacy gap is crucial for maximizing the potential of eHealth.
