1) It is immunization day at the health center. The first patient is a 2-month old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta) and oral polio vaccine. PCV and Penta are given via intramuscular route. Where are the sites of injection? What muscle is targeted in the injection? Write down its origin, insertion and action. How do you locate this area?

 

Site of Injection: Anterior and lateral part of the thigh (Anterolateral thigh)

Muscle: Vastus Lateralis

Origin: Linea aspera of the femur and the great trochanter

Insertion: Tibial tuberosity through the  patellar ligament and patella via quadriceps tendon

Action: Extends the leg at the knee joint; the rectus femoris, when acting alone, also flexes the thigh at the hip joint.

Location: Position the baby with the thigh exposed, find the middle third of the thigh, located between the greater trochanter and the knee. Give the injection in the front outer part of this section of the thigh.

 

 

 

2) The baby’s 65-year-old grandfather also came to the center because influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly. Which muscle is the preferred site for IM injection in this case? Write down its origin, insertion and action. How do you locate this area?

 

Site of Injection: Deltoid muscle in the upper arm

Muscle: Deltoid

Origin: Acromial extremity of clavicle (anterior fibers), acromion of scapula (lateral fibers), and spine of scapula (posterior fibers)

Insertion: Deltoid tuberosity of humerus

Action: The shoulder joint can move backward, forward, and away from the body

Location: Have the patient relax their arm by their side. Locate the prominent bone at the top of the shoulder. Administer the injection at the thickest area, which can be found by measuring two to three finger widths down the lateral side from the acromion process.
