The following are some of the challenges in clinical teaching which I personally encounter

1. Availability of the Clinical instructor. at times due to some emergency situation, the clinical instructor has to leave his class. To address this, the instructor can provide a video lecture or ask the most senior resident to take over. Then an output may be asked from them like a video recording of their output to which the clinical instructor can check later.

2. Availability of a target px case.  there are times that we need a particular px to discuss. however, during the time of rotation, no px is available. In order to address this, we can do case scenario, role playing where we ca discuss the target topic. example, if the topic is to explain the contemplated procedure to the relatives and patients, a simulated case or a role playing can be done where the students will have to demonstrate proper explanation of the procedure as well as discussing the risk of the surgery.

3. uncooperative patients.  at times there are patients and relatives who are uncooperative to the students and doctors. They do not want to talk to the students because they feel they were being "just studied" by them. To address this, the instructor can bring the students with him and together, they will see and talk to the relatives and patients. The instructor will do a side by side teaching, at the same time role modeling. 

4. unavailable px for Operation. In MIS rotation, residents should be able to demonstrate their learning by assisting the surgeon. To address this, they can do it in the training box where they can do simulation like cystectomy together with the surgeon. They can practice the things they will have to do once px is available already.

Uncooperative patients are commonly encountered maybe due to fatigue from repeated history taking and examination from medical students, interns and residents. Proper timing and building rapport and empathy with the patient might improve the situation.
