These are some of the challenges and possible solutions in my specific clinical setting

1. No prior knowledge of students on the topic for discussion - give reading assignments prior to the actual patient encounter (planning ahead) and do short brainstorming activities prior to the activity itself

2. Informant giving disorganized data - assign students in pairs with teacher supervision to obtain the data and information needed

3. Few patients for the assigned unit/area - use of simulation and medical models

4. Language barrier of students to informants/patients - assign students in pairs with one student fluent in the local dialect and with teacher supervision 

5.No medical instruments to use - assign a specific student to bring the needed medical instruments

I agree with you, Weng, especially #3-5. During the pandemic, we have done role-playing for history taking with the preceptor acting as the patient; this can still be an excellent alternative strategy for a few patients on the ward. A language barrier between patient and student sometimes is becoming typical, and I agree to pair this student with those fluent in the local language. For the lack of resources the school provides, ensuring that all students bring their instruments is another option if these are affordable. Still, if not, then we can just have a simulation using improvised materials. Thank you Weng.

True, role playing has been one of the favorite activity during the pandemic. Everyone became actors and actresses in order to facilitate discussion and to emphasis things that the residents and students can appreciate.
