  1. It is immunization day at the health center. The first patient is a 2-month old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta) and oral polio vaccine. PCV and Penta are given via intramuscular route. Where are the sites of injection? What muscle is targeted in the injection? Write down its origin, insertion and action. How do you locate this area?
  • SITE: Anterolateral thigh of opposite legs
  • TARGETED MUSCLE: vastus lateralis
  • ORIGIN: Greater trochanter and linea aspera of femur
  • INSERTION: Patella via the quadriceps tendon; then tibial tuberosity via the patellar ligament
  • ACTION: extend the lower leg and allow the body to rise up from a squatting position.
  • HOW TO LOCATE: 
    • To find the thigh injection site, make an imaginary box on the upper leg. Find the groin. One hand’s width below the groin becomes the upper border of the box
    • Find the top of the knee. One hand’s width above the top of the knee becomes the lower border of the box

2. The baby’s 65-year-old grandfather also came to the center because influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly. Which muscle is the preferred site for IM injection in this case? Write down its origin, insertion and action. How do you locate this area?

    • SITE: upper arm
    • TARGETED MUSCLE: deltoid
    • ORIGIN:  third of Clavicle (clavicular part), Acromion (acromial part), Spine of Scapula (spinal part) 
    • INSERTION: deltoid tuberosity of humerus 
    • ACTION: 
      • Clavicular part: flexion and internal rotation of the arm,
      • Acromial part: abduction of the arm beyond the initial 15°
      • Spinal part: extension and external rotation of the arm.
    • HOW TO LOCATE: 
      • Draw an imaginary inverted triangle between the acromion and the deltoid tuberosity. The correct injection site is in the middle one-third of this triangle (ie, in the center of the deltoid muscle, midway between the acromion and the deltoid tuberosity).
