Exercise #10A: Proof of Submission of Research Proposal to UPM REB (Group Task)
Upload PDF copies of the pertinent files related to the submission of your Group’s research proposal to UPM REB for ethical approval:
1. research proposal with appendices
2. filled-out and signed UPM REB forms
3. acknowledgment e-mail from UPM REB that your research proposal is for classification (sent after iREB submission and verification)
Use the following filename format:
· DENT198_FINAL Research Proposal_GROUP X
· DENT198_UPMREB Form 2X_GROUP X
· DENT198_UPMREB Acknowledgment_GROUP X
All the uploaded files must also be sent via-email to the entire Research Team (CC: mfmendoza3@up.edu.ph) using the subject line:
[DENT198] Proof of UPM REB Submission_GROUP X
REMINDERS:
1. iREB submission is not considered an official UPM REB submission as submission to iREB is subject to verification and assessment of completeness of submitted e-documents. Submissions are considered officially submitted when acknowledged by UPM REB via e-mail stating that the submitted research proposal is for classification.
2. The filled-out and signed UPM REB forms to be uploaded must include ALL pages of ALL the forms submitted to UPM REB.
TARGET DUE DATE varies depending on your scheduled presentation date
Note that students must submit this as a group. ALL group members must click the submit button for it to be officially submitted.
ADDENDUM:
Please be reminded that proof of submission of your Group's research proposal must be submitted/uploaded to be given a Final Grade for Dentistry 198. With the health break and work suspension in UP Manila from 14-21 January 2022, the deadline to submit Dent198 requirements to be issued a Final Grade is further moved to Monday, 31 January 2022, 11:59PM.
