🎗 For the Block LO kindly ask your block mates to accomplish the CIF (in the templates) and upload to the appropriate GDrive folder for documentation (and possible processing). Please accomplish by Rotation Day 04 (Sunday). Target testing date is Rotation Day 06 (Tuesday; 8AM to 12NN protected time). THE BLOCK LO SHOULD REACH OUT TO UPHS FOR PROCESSING OF THE BLOCK CIFS AS WELL AS SCHEDULING OF TESTING. The FCH 260.2 coordinator will reach out to the UPHS as a redundancy BUT it is the block LO's responsibility to facilitate the scheduling of their block mates according to the UPHS protocols.

🎗 Reminder also to accomplish the PhilHealth claim signature form (in the templates). Please take note that It will need to be printed out and brought to the OP swab facility on Wednesday (so that your swab will be charged to PhilHealth). :)

📌 Please wait for the UPHS to contact/call you (latest Monday afternoon, Day 05) for the testing schedule and instructions. If by 3PM (Monday), there is still no word from UPHS, kindly inform the FCH 260.2 Course Coordinator so we can follow them up.

🎗 You also need to accomplish the Informed Consent Form (also in the document templates). As the FCH 260.2 Course Coordinator will explain during the Orientation, you will be the ones signing the form but please make sure to show the form (and discuss it) with your parents/guardians so that they are aware of the nature of the rotation. Once signed please upload them to this folder (no informed consent form, no deployment):
