From time to time, you may need to update your nursing care plan to determine if goals and outcomes are achieved, and evaluate the patient’s condition for improvement or worsening. This process will help you identify whether you need to modify, continue or terminate the plan of care, identify other needed resources, or determine what the health care team need to discuss and address. On page 1062 of Berman et al.’s (2016) 10th edition of Kozier & Erb's Fundamentals of Nursing, you can see a concept map displaying the application of the nursing process to care for patient at risk for disuse syndrome.
