  1. Infant Immunization: The first patient is a 2-month-old healthy female infant scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta), and oral polio vaccine. Both the PCV and Penta vaccines are administered via the intramuscular route.
  • Injection Site and Muscle: For a 2-month-old infant, intramuscular injections are typically administered in the anterolateral part of the thigh, targeting the Vastus lateralis muscle, which is part of the quadriceps group.
    • Muscle Details:
      • Origin: Greater trochanter and linea aspera of the femur.
      • Insertion: Patella via the quadriceps tendon, then tibial tuberosity via the patellar ligament.
      • Action: All four heads of the quadriceps extend the leg at the knee joint, and the rectus femoris muscle also flexes the thigh at the hip joint.
  • Locating the Injection Site:
  1. Divide the area from the greater trochanter to the lateral femoral condyle into thirds.
  2. Palpate the center of the middle third and locate the outer anterolateral aspect, lateral to the midpoint of the thigh.

      • Origin: Acromial extremity of the clavicle (anterior fibers), acromion of the scapula (lateral fibers), and spine of the scapula (posterior fibers).
      • Insertion: Deltoid tuberosity of the humerus.
      • Action: Lateral fibers abduct the arm at the shoulder joint; anterior fibers flex and medially rotate the arm at the shoulder joint; posterior fibers extend and laterally rotate the arm at the shoulder joint.
  • Locating the Injection Site: The injection should be administered in the thickest part of the deltoid muscle, approximately 2 inches (5 centimeters) below the acromion process.
