1) It is immunization day at the health center. The first patient is a 2-month old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta) and oral polio vaccine. PCV and Penta are given via intramuscular route. Where are the sites of injection? What muscle is targeted in the injection? Write down its origin, insertion and action. How do you locate this area?
Patient: 2-month old well female infant
Site of injection: Anterolateral portion of the thigh
Targeted Muscle: Vastus Lateralis
Origin: Lateral intermuscular septum, lateral lip of linea aspera and gluteal tuberosity
Insertion: patella, medial patellar retinaculum
Action: extends leg
How to locate the area:
1.Infant must be placed and rested in a supine position or held in a supine position.
2.Palpate between the greater trochanter and lateral femoral condyle. The site of injection is between the two sites.
3. Divide the muscle into thirds. The injection should be administered on the middle third of the muscle between the greater trochanter and lateral femoral condyle, in the outer anterolateral aspect, lateral to the midpoint of the thigh
2) The baby’s 65-year-old grandfather also came to the center because influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly. Which muscle is the preferred site for IM injection in this case? Write down its origin, insertion and action. How do you locate this area?
Patient: 65-year-old grandfather
Site of injection: Upper arm
Targeted Muscle: Deltoid
Origin: Lateral one-third of clavicle, acromion, lower lip of the crest of the spine of scapula
Insertion: Deltoid tuberosity of humerus
Action: Abducts arm, extension of posterior fibers and laterally rotates arm, flexion of anterior fibers and medially rotates arm
How to locate the area:
1.Patient must be sat down with their arms relaxed.
2.Palpate the acromion process at the top of the shoulder
3.The injection site is approximately 2-3 fingerbreadths below the acromion process and middle of deltoid muscle.
