Hello, Dr. Sumile! I would like to clarify the following:

1. I tried to group the signs and symptoms of hypo/hypernatremia to better understand their causes (Image 1). Please correct me if I got any of them wrong. 

a. Is the nausea likely caused by potential increased ICP?

b. In hypernatremia, how come the tongue gets swollen and the mucous membranes become sticky? Based on my understanding, the epithelial cells should shrink due to the shift of fluid from the inside to the outside. 

I do acknowledge that a lot of manifestations tend to overlap, so it's important to correlate them within the patient's context. I hope I'm not overthinking these 😅

2. (Image 2) In documenting the presence/absence of phlebitis and infiltration, do we record the signs and symptoms as is, or is it still clinically relevant to use the attached grading (I saw them while browsing Brunner & Suddarth's 12th ed. published in 2009)?
