1) It is immunization day at the health center. The first patient is a 2-month old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta) and oral polio vaccine. PCV and Penta are given via intramuscular route.

  • Where are the sites of injection?
    • Anterolateral part of the thigh
  • What muscle is targeted in the injection?
    • Vastus lateralis
  • Write down its origin, insertion and action.
    • Origin - The greater trochanter and linea aspera of the femur
    • Insertion - The patella through the quadriceps tendon and the tibial tuberosity through the patellar ligament
    • Action - Extends the leg at the knee joint
  • How do you locate this area?
    • Examine and/or palpate the muscle that lies on the axis of the septum dividing the vastus lateralis and the rectus femoris muscles. 

2) The baby’s 65-year-old grandfather also came to the center because influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly. 

  • Which muscle is the preferred site for IM injection in this case?
    • Deltoid
  • Write down its origin, insertion and action.
    • Origin - The lateral part of the clavicle, spine and acromion process of the scapula 
    • Insertion - The deltoid tuberosity of the humerus
    • Action - Abducts the arm at the shoulder joint (lateral fibers), flex and rotate the arm medially at the shoulder joint (anterior fibers), and extend and rotate the arm laterally at the shoulder joint (posterior fibers)
  • How do you locate this area?
    • Examine and/or palpate the muscle outside of the upper arm, on top of the humerus, the clavicle, and the scapula.
