1) It is immunization day at the health center. The first patient is a 2-month old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta) and oral polio vaccine. PCV and Penta are given via intramuscular route.

For PCV - Right Anterolateral Thigh - Vastus lateralis Muscle
For Penta Vaccine - Left Anterolateral Thigh - Vastus lateralis Muscle
What muscle is targeted in the injection?
Vastus lateralis
Origin: Greater trochanter, Lateral lip of linea aspera
Insertion: Quadriceps tendon and tibial tuberosity
Action: Extension of leg at knee joint
How do you locate this area?
Visualize dividing the infant's thigh on three parts from the Greater Trochanter up to the Lateral Femoral Condyle of knee. Inject the vaccine to the middle third (middle of 2/3) portion.
2) The baby’s 65-year-old grandfather also came to the center because influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly.
Which muscle is the preferred site for IM injection in this case?
Deltoid muscle
Origin: Clavicle, Acromion of scapula, Spine of scapula
Insertion: Deltoid tuberosity of humerus
Action: Flex and medially rotate arm; Abducts arm; Extend and laterally rotate arm
How do you locate this area?
Visualize an inverted triangle, which extends from base of acromion process and extends down to a point level with axilla. Administer the vaccine in the middle of the inverted triangle.
