This form is for evaluation of your consultant interactions e.g. when you went on service rounds, when you endorsed but were unable to present a full case, etc. At the end of the year you will need at least 2 of these evaluations.
This form is for the evaluation by your ward service/duty resident. At the end of the year you will need at least 1 junior and 1 senior IM resident evaluation
This form is used by consultants for your evaluation when you present a full case. May be done during OPD case presentation, LU ward rounds or service rounds. This will also serve as the rubric for the end of the year case presentation. At the end of the year you will need at least 2 of these evaluations.
