Course Description for ORL 251 SY 2022-2023
Warm Welcome to ORL 251! We hope you will have a fruitful learning experience from this course.
Traditionally ORL 251 is an intensive 2-week course for Learning Unit 6 students where they integrate and apply concepts learned in history taking, physical examination, diagnosis and management of ORL patients at the ward, emergency room, major operating room, minor operating room and out-patient clinic. Students are also exposed to the specialty clinics - LBEN, Vertigo, Ear, Calorics, Rhino, CIOPS and Craniomaxillofacial-Plastic where they encounter the not-so-common and complicated cases.
In light of the COVID-19 pandemic, this modified curriculum is intended to help augment the learning of UPCM LU6 students in the clinical application of concepts learned through LU3-LU5 in spite of the limitations in hospital and face-to face patient exposure.
This modified curriculum is divided into two separate one-week rotations in order to give the students the best chance to have face-to-face clinical exposure given the uncertainty in quarantine restrictions due to the covid-19 pandemic. The rotations consist of both face-to-face (when allowed) and remote/blended activities.

Over-all general guidelines for LU6 students have been provided for by the LU6 Committee of the College of Medicine and are to be followed accordingly. Similarly any updates from UP-PGH, or the HICU regarding covid 19 protocols, personal protective equipment are to be followed accordingly and supercede instructions in this document.
Additionally, we also provided remote/online teaching content to supplement the student's learning experience in the UPM VLE ORL 251 SY 2022-23 Course. These include the following:
1) Asynchronous didactic modules on key topics in ORL to be completed by the students anytime prior to the completion of the academic year. Completion of these modules will comprise 5% of each student's grade. Content of these modules may also be included as part of the end of year exam and comprehensive examination.

• Foreign Bodies in the Esophagus and Airway
• Head and Neck Malignancies (SCCAs)
• Surgical Management of Voice Disorders
• Facial Fractures
• Temporal Bone Trauma
• Penetrating and Blunt Neck Trauma
• Laryngotracheal Stenosis and Tracheostomy
• Obstructive Sleep Apnea
2) Remote clinical integration through:
• Access to selected previous ORL department grand rounds case presentations, subspecialty lectures, reading materials
• Invited attendance to the weekly departmental census and grand rounds presentation currently being held via zoom web-conference or in-person (Friday morning)
• Group powerpoint slideshow presentation discussion of 1 clinical case each week via synchronous zoom-teleconference or in-person preceptorship (to be held Friday or Saturday depending on agreement with consultant of the week)
• Alternately, as a low-bandwith option for students with limited access to internet connectivity precluding participation in the zoom group session - students may opt to instead submit individual word document file papers answering the guide questions for the case.
ORL 251 Grading Rubrics:
• Completion of Asynchronous Didactic Modules within the academic year (5%)
• Attendance, Punctuality, and Performance in Hospital Clinical Posts
o Due to uncertainty brought about by the COVID-19 pandemic in the ability to ensure all students are able to have face-to-face rotations the UPCM LU6 committee has decided not place weight in grading to the clinical rotations.
o Attendance however will still be monitored for completion
o Administrative sanctions may also be given if warranted depending on incident severity.
• Case Discussion (25%)
o Group Zoom Case Presentations or Individual Papers Submission
o 2 Cases (12.5% each)
• End of year ORL 251 Final Exam (60%) and LU6 Comprehensive Examination (10%)
I. ASYNCHRONOUS DIDACTIC MODULES
Below are several didactic modules designed by the Consultants of Department of ORL. These have been created or curated to cover topics relevant to ORL in LU6.

These are asynchronous learning activities which you can go though at your own pace anytime within your ORL 251 rotation but will be available throughout the entire academic school year to give ample opportunity for students with poor connectivity. You will be evaluated based on completion of the modules as documented through your recorded activities in the UPM VLE LMS. Completion of the modules will constitute 10% of your ORL 251 grade.
Additionally, these modules may also be included as part of the topic coverage of your end of rotation and/or end or the year examination.

II. REMOTE CLINICAL INTEGRATION
In order to augment the limited face-to-face clinical exposure due to the COVID-19 pandemic and community quarantine, we have prepared the following activities:
- Access to a repository of selected recent prior grandrounds presentations, lectures, and learning material
- Access to the weekly departmental zoom webconference summary rounds and grandrounds presentation
- Clinical paper case discussion via zoom webconference powerpoint slideshow small group discussion or individual word document submission for grading and feedback with an ORL consultant preceptor.
Details of each activity are further discussed below.
A. ORL-PGH Grand Rounds Case Presentations and Lectures
The ORL department has compiled below several recent grand rounds case presentations as well as selected lectures and learning material for your review. The content of these presentations may be included in your end-of-rotation as well as end-of-the-year examinations. If you have any questions or clarifications regarding the cases or learning material, you may reach out to the ORL LU6 resident and consultant preceptor/coordinator.
B. Participation in the Department of ORL Weekly Summary Rounds & Grandrounds
Every friday morning at 8am the Department of ORL conducts a summary rounds presentation of all cases managed by the department for the week. This is usually followed by grandrounds presentations of one pre-operative case and one post-operative case. Currently these activities are held via zoom webconference or in-person with the department consultants, residents, and rotating students in attendance.
LU6 students are requested to be present during these departmental activities, and may be called upon to react or answer theoretical questions relevant to the cases presented. You may ask your resident coordinators for the diagnosis of the cases to be presented as well as for the zoom invitation link.
For students with limited internet connectivity, you can inform your resident coordinators beforehand if you foresee inability to join this activity or if you have problems connecting during that morning. Copies of the grandrounds presentations may be provided to these students upon request.
Due to the limited face-to-face clinical exposure time provided by the current circumstances, we highly recommend all LU6 students to participate in this activity.
C. Clinical Paper Case Assignments
Below are six clinical paper cases. In case face-to-face rotations are not allowed (instead of a ward case) each rotating LU6 block is to select one of the six clinical paper cases and present and discuss the cases and their corresponding guide questions via powerpoint presentation and group discussion during a zoom web-conference small group discussion with their ORL consultant preceptor. Unless otherwise specified, this preceptorial will be held on the Friday or Saturday of the Rotation Week based on mutual agreement with the ORL consultant preceptor of the week.
- The ORL resident monitor will assist in coordinating this synchronous online small group discussion.
- This format would allow for the most optimal opportunity for interactive discussion and immediate feedback between the rotating block and the ORL consultant preceptor.
- Each student would be graded individually based on the content of the presentation as well as in their participation in the discussion.
For students with limited connectivity who may have difficulty in participating in this synchronous online activity, they may alternately opt to submit individually one of the six clinical paper cases and answers to guide questions via word document for grading and feedback by their ORL consultant preceptor. Deadline of submission individually submitted paper case discussions to the consultant preceptor should coincide with the start time of the synchronous zoom webconference group presentation and discussion.
All power point presentations and clinical paper case word document submissions must also be emailed to orl251.202223@gmail.com in order for the assignment to be deemed completed. This will provide a compiled repository of all group presentations and individual paper submissions.
Students are firmly reminded to prepare and submit original work (group power point presentations and individual word document papers) without influence from work done by prior rotating blocks and students. Appropriate referencing of sources must also be cited in the presentations and papers. Student work with evidence of plagiarism (from prior students' work or from uncited sources) will be subject to disciplinary action based on the codified rules of the College of Medicine.
In case both rotation weeks of the block are via remote/online (non-face to face), they are to select a different paper case for their second week preceptorial. The block or students cannot repeat the same case for both preceptorials.
This activity will constitute 50% of each students' ORL 251 grade. 25% allocated for each of the two selected clinical cases.
