How does the food change in consistency and form?

The food we ingest changes in consistency and form in multiple stages as it passes through our GI tract. First, it gets broken down into smaller, more manageable pieces by the mouth with the help of enzymes present in the saliva. It then travels to the stomach where the stomach muscles churn and mix the food with digestive juices that have acids and enzymes that help break up food into much smaller pieces and are turned into a thick liquid called chyme when it is ready to leave the stomach. It then travels through the small intestine where water and digested nutrients are absorbed and returned to the body's bloodstream. The absorption of liquid in this stage causes the consistency of food turn into a more dense, firmer substance. 

How could the body absorb the nutrients from the foods we eat?

When food is squeezed into the jejunum and ileum, nutrients are then absorbed from the ileum which is lined with villi, which is then connected to a mesh of capillaries where nutrients pass into the bloodstream after absorption.
