  1.  It is immunization day at the health center. The first patient is a 2-month-old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta), and oral polio vaccine. PCV and Penta are given via the intramuscular route. Where are the sites of injection? What muscle is targeted in the injection? Write down its origin, insertion, and action. How do you locate this area?

  1. SITES OF INJECTION:

  • PNEUMOCOCCAL CONJUGATE VACCINE: Right anterolateral aspect of mid-thigh

  • PENTAVALENT VACCINE SITE OF INJECTION: Left anterolateral aspect of mid-thigh

 

  1. TARGETED MUSCLE: Vastus lateralis

  • ORIGIN: Greater trochanter and femur’s linea aspera 

  • INSERTION: Patella via the quadriceps tendon and the tibial tuberosity via the patella ligament.

  • ACTION: Leg extension at the knee joint

 

  1. HOW TO LOCATE THE AREA: Palpate the muscle above the lateral femoral condyle and beneath the greater trochanter. By dividing the muscle into thirds, the outer anterolateral aspect of the muscle in the middle third, lateral to the midpoint of the thigh, is the ideal location for injection. 

 

  1. The baby’s 65-year-old grandfather also came to the center because the influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly. Which muscle is the preferred site for IM injection in this case? Write down its origin, insertion, and action. How do you locate this area?

  1. SITE OF INJECTIONMuscle of the upper arm

 

  1. TARGETED MUSCLE: Deltoid

  • ORIGIN: Clavicular acromial extremity (anterior fibers), scapular acromion (lateral fibers), and the scapular spine (posterior fibers).

  • INSERTION: Deltoid tuberosity of the humerus

  • ACTION: Extending and lateral rotation of the arm at the shoulder joint (posterior fibers), flexing and medial rotation of the arm at the shoulder joint (lateral fibers), and abducting of the arm at the shoulder joint (lateral fibers).

 

  1. HOW TO LOCATE THE AREA: Palpate the bony acromion process, then position three fingers beneath it and across the deltoid muscle to pinpoint its location.
