1) It is immunization day at the health center. The first patient is a 2-month old well female infant who is scheduled for the following vaccinations: pneumococcal conjugate vaccine (PCV), pentavalent vaccine (Penta) and oral polio vaccine. PCV and Penta are given via intramuscular route. Where are the sites of injection? What muscle is targeted in the injection? Write down its origin, insertion and action. How do you locate this area?

  • Site of injection: Anterolateral thigh
  • Targeted muscle: Vastus lateralis muscle
    • Origin: Greater trochanter and linea aspera of femur
    • Insertion: Patella via quadriceps tendon and then tibial tuberosity via patella ligament
    • Action: Knee flexion, and extend leg at knee joint.
  • How to locate this area:
    1. Have the patient lie flat on their back (supine). 
    2. Horizontally and vertically divide the thigh into thirds, and apply the injection at the outer middle third.
    3. Feel for their greater trochanter and rest your hand or pinky on it while spreading your fingers.
    4. Locate the popliteal immediately below the knee with your other hand. Place your hand on it and separate your fingers.
    5. The vastus lateralis should be between your two thumbs; if it cannot be located, have the patient dorsiflex their foot to protrude the vastus lateralis muscle.

2) The baby’s 65-year-old grandfather also came to the center because influenza vaccine is being offered to senior citizens. The flu vaccine is given intramuscularly. Which muscle is the preferred site for IM injection in this case? Write down its origin, insertion and action. How do you locate this area?

  • Site of injection: Upper arm
  • Targeted muscle: Deltoid
    • Origin: Acromial extremity of clavicle (anterior fibers), acromion of scapula (lateral fibers), and spine of scapula (posterior fibers).
    • Insertion: Deltoid tuberosity of humerus.
    • Action: At the shoulder joint, lateral fibers abduct the arm, anterior fibers flex and medially rotate the arm, and posterior fibers lengthen and laterally rotate the arm.
  • How to locate this area:
    1. Locate the deltoid muscle by palpating the acromion process's lower edge.
    2. At the middle in line with the axilla on the lateral side of the upper arm, a triangle is formed, with its base at the acromion process.
    3. Injection should be administered three finger widths below the acromion process.
