Scenario: For your N11 Clinical Duty, you were assigned to go on duty at the medical ward.
Your client is Linda , a 45-year-old woman who presented to the ER of the Philippine General Hospital with complaint of difficulty of breathing . Her daughter claims she has episodes of disorientation to time, person and place.She was just admitted to the medical when you started your duty. Upon assessment, she presented with the following signs and symptoms:
•Orthopneic; alar flaring; use of accessory muscles; RR = 40/min; with rales at both lung fields, mid to base
•With complaint of chest pain, especially upon inhalation; HR = 96/min, regular
•with complaint of easy fatigability; claims that she has problem doing activities of daily living without assistance
•noted to be restless; disoriented to person and place
. poor appetite
• with circumoral cyanosis
•On Oxygen 4l/min via nasal cannula
•Dry cough
•Known COPD
•+ clubbing of fingers; capillary refill time-4 secs
1. Identify 2 nursing diagnosis and prioritize
2. Make an NCP for the identified nursing problems
